    Beautiful but unsafe, inconsistent care

I'm torn: the facility is beautiful, clean, with great food, frequent activities and many kind therapists and staff. But chronic understaffing, constant CNA turnover and too many managers produced inconsistent, sometimes horrendous care - my loved one was left soiled for 16 hours, went 14+ hours unattended, experienced falls, heavy meds and wheelchair confinement, and meals were skipped. Management was unresponsive; lovely environment but not reliable for anyone needing real assisted or memory care.

    Summary review

Overall sentiment is mixed and polarized: many reviews praise the staff, therapists, cleanliness, and the pleasant property, while an overlapping set of reviews raises serious concerns about staffing, management, and episodes of neglect. The collection of summaries points to a facility that can offer strong, compassionate care and effective therapy for some residents but also exhibits systemic problems that have led to significant negative experiences for others.

Staff and care quality form the clearest pattern of divergence. Multiple reviewers describe the staff as caring, friendly, loving, and professional; there are repeated compliments for therapists and teams that provide effective treatment and strong support. Several accounts state residents are happy, well cared for, and that the atmosphere is upbeat. Conversely, an equally notable subset of reviews reports chronic understaffing, a revolving door of CNAs, and high employee turnover. These staffing issues are tied in the reviews to serious lapses in care: described incidents include residents being left in soiled conditions for many hours (examples given of 14+ and 16 hours), falls, skipped meals, and what some reviewers call horrendous care. Some reviews accuse the facility of overmedicating residents or confining them to wheelchairs, suggesting concerns about both neglect and inappropriate medication practices.

Management and communication issues emerge repeatedly. Reviewers note an apparent imbalance between administrative staffing and floor-level caregivers—“too many managers” vs “not enough workers”—and describe management as unresponsive or failing to apologize after incidents. A resident advocate is described as unresponsive in at least one summary. There are also allegations about misleading or false claims related to memory care. These themes together suggest inconsistent oversight and gaps in accountability that reviewers link to the poor outcomes they experienced.

Activities, dining, and facility conditions are described inconsistently. Several reviews praise frequent programming (2–4 activities daily), church services, and a range of social opportunities; others say activities are passive or superficial (for example, leaving residents to watch a fish tank). Dining reviews vary widely: some call the food “awesome,” while others say the kitchen was closed at times or that food quality was poor and meals were skipped. The physical plant is similarly described in two ways—reviewers praise a clean, upbeat environment with large visiting areas and attractive grounds, but others note the building is old and in disrepair. This contrast suggests that while common areas may be well-maintained and the grounds pleasant, certain infrastructural or operational issues remain.

A notable pattern is inconsistency: many positive comments coexist with severe negative incidents. The mixed reports imply variability by shift, wing, or timeframe—when staffing and management are effective, residents receive high-quality care and support; when turnover is high or staffing is thin, care declines and serious problems can occur. Given the specific reports of extended neglect and management unresponsiveness, these are red flags for families assessing the facility.

In summary, San Rafael Health and Rehabilitation (Emery County Care & Rehab Center) appears capable of providing compassionate, high-quality rehabilitation and long-term care for many residents, with strong therapists, friendly staff, good communal spaces, and pleasant grounds. However, recurring and serious complaints about understaffing, staff turnover, administrative imbalance, long neglect episodes, inconsistent meals and activities, and occasional lack of empathy or responsiveness from management create a risk of poor outcomes for other residents. Prospective residents and families should weigh both sets of reports, ask targeted questions about current staffing levels, turnover, memory-care specifics, recent incident reports, meal schedules, activity programming, and complaint resolution, and conduct in-person visits across different shifts to better assess the facility’s consistent day-to-day performance.
