    Unclean, unsafe care; occasional compassion

I had a mixed but mostly troubling stay. The place was often dirty - urine/feces odor, bugs, stained linens and floors not mopped. Staff were frequently unprofessional, loud, ignored call bells and privacy, and I saw neglect: missed or wrong meds (even critical ones), unattended patients, falls and delayed responses. Management and phones were unresponsive, though a few people (notably Ms. Baker, some nurses and therapists) were compassionate, coordinated care well, and rehab helped. I can't recommend this facility until cleanliness, staffing and safety are fixed.

    Summary review

Overall sentiment in the reviews for Annandale Healthcare Center is highly polarized, with strong, repeated praise for specific staff members, therapy services, and some administrative functions alongside frequent, serious allegations of neglect, poor sanitation, unsafe care, and management failures. Many reviewers describe excellent experiences centered on compassionate individual caregivers, effective rehabilitation (PT/OT/Speech), and especially exceptional casework and advocacy provided by a repeatedly named staff member (Emirror / Ms. Baker). These positive reports often emphasize clear communication, helpful admission and discharge assistance, successful rehab outcomes, and staff who treat residents like family. Several families credit the facility with meaningful medical improvements, good pain management, coordinated care with physicians, and strong assistance navigating VA benefits or insurance processes.

However, an equally large and concerning body of reviews reports systemic and severe negative experiences. The most frequently cited problems are unreliable staff responsiveness to call lights and phones, reports of residents left soiled or unattended, long delays in assistance, and in multiple reports, alleged physical abuse or rough handling. There are specific and alarming allegations of theft (cash, narcotics, electronics), missing or incorrect medications, failures to provide life-sustaining items (oxygen, transplant meds), and instances where emergency response was inadequate or delayed. Several reviewers describe incidents resulting in hospitalization, sepsis, bedsores, or even death, and note delayed notification of family members. Together these issues raise significant safety and quality-of-care concerns.

Facility conditions and housekeeping are another major theme of criticism. Multiple accounts describe urine and feces odors in corridors and bathrooms, soiled linens, cockroaches, dirty floors, stained sheets, malfunctioning beds, and generally outdated or run-down building areas. While some reviewers praise a clean, renovated rehab wing and improved areas under remodeling, the contrast between cleaner rehab spaces and neglected long-term areas is a recurrent pattern. Housekeeping lapses extend to infrequent showers, irregular bathroom cleaning, and failure to change curtains or clean personal equipment.

Dining, activities, and resident life are inconsistently reported. Several families praise engaging activities, responsive activities staff, and flexible dietary accommodations, and a number of reviews mention good meals. Conversely, many reports say meals arrive cold, lack hydration (no water offered), poor meal quality, limited programming in some parts of the facility, crowded rooms, and minimal stimulation for residents. Staff behavior and professionalism are likewise mixed: quite a few reviews commend kind, patient, and hardworking nurses and CNAs, while many others report rudeness, privacy violations (not knocking), staff on phones or lounging, and management that fails to address complaints.

Management and communication exhibit stark variability by account. Numerous positive reviews point to efficient admissions, responsive caseworkers, and new managers who are making improvements. Recurrent praise for specific staff—especially the named caseworker—highlights the impact of strong staff advocates on family satisfaction. At the same time, a substantial number of reviewers describe poor leadership, lack of accountability, slow or missing follow-up to complaints, inaccurate discharge paperwork, billing disputes, and calls for staff replacement. Several reviews explicitly recommend avoiding the facility or seeking alternative placements due to perceived systemic failures.

Notable patterns and risk signals: safety incidents (falls, delayed discovery), medication and equipment lapses, alleged abuse and theft, and sanitation failures appear in multiple, independent accounts and therefore are not isolated complaints. The coexistence of high-quality rehab services and praised individual caregivers with these safety and cleanliness problems suggests uneven standards across departments and shifts. Some reviewers report significant improvements under recent management changes and remodeling, which indicates that experiences may be changing over time and that quality may depend heavily on which unit, wing, or staff members are involved.

In summary, Annandale Healthcare Center receives mixed reviews with polarized experiences. Strengths include standout individual staff members, solid rehab services, helpful admissions and casework, and positive outcomes for some residents. Serious and recurring weaknesses include safety and neglect concerns, poor responsiveness, sanitation and hygiene failures, medication and theft incidents, and inconsistent leadership and communication. Prospective residents and families should weigh the frequency and severity of the negative reports against the positive testimonials, visit in person, inquire specifically about staffing, call systems, incident reporting, and sanitation protocols, and verify caseworker availability. The reviews indicate that while excellent care is possible at this facility, there are documented and recurring risks that merit careful consideration and active oversight by families and referring clinicians.
