E. A. Hawse Healthcare Center sits in Baker, West Virginia, and has served the community since 1992, operating with 60 certified and licensed beds, providing skilled nursing care, short-term and long-term rehab, and therapy for those who need extra help. Families can take part in planning care, and staff try to build good relationships, making the place feel welcoming while focusing on comfort and happiness, though inspection reports have found 21 deficiencies, including infection-related issues and problems with food handling and safety hazards, both marked with severity E. The nurse turnover rate is high at 98.2%, with staffing averages about 3.23 nurse hours per resident per day. Even so, residents find access to all sorts of therapy-physical, occupational, speech-and benefit from specialty services like wound care, palliative care, and hospice, with the center providing person-centered treatment for both rehab and memory care needs. The outdoor spaces give a place to relax, and everyday life includes community activities such as bingo and holiday celebrations. Nutritional needs get attention, though inspectors noted some past problems with sourcing and food safety. The center runs under CommuniCare Health, which manages many such facilities across the country, though Route 55 Leasing Co., LLC holds the legal name. Administrator Pauline Vance oversees the day-to-day, and staff speak English. E. A. Hawse is dual-certified for Medicare and Medicaid, with options for private pay, and offers an in-house pharmacy where residents can get prescription medications at lower costs. The center specializes in services for seniors needing skilled care, long-term stays, or memory support, and offers medical, dental, behavioral, and pharmaceutical care under one roof. The facility has not been accepting new patients lately, but staff can give information about when beds might open up, and all in all, their approach is meant to be supportive, community-focused, and tailored to each person's needs, even while navigating the challenges that come with healthcare today.

Neglectful care, premature discharge, unsanitary
I was deeply disappointed with my rehab - PT/OT couldn't manage my COPD, staff failed to recognize how ill I was, I was discharged prematurely, had to be put in an ambulance and spent another week in hospital, and only recovered at home with home services. Care felt neglectful: an emphasis on occupancy over patient-focused care left me unsanitary and ignored (no bed baths, no face/oral care, nails not cleaned), persistent odor, small room, slow responses, poor night staffing, no activities and horrible communication with family. The grounds, rooms and some staff were nice and the manager seems on top of things, but the lapses in clinical care, cleanliness and responsiveness made it unacceptable for me.
