    Ignored alarms, poor nursing care

I cannot recommend this facility right now. I saw emergency call buttons ignored for hours, meds missed or incorrect, staff bickering and laughing while patients went unattended (one needed ER transport), soiled bedding/odors, cold or late food, and safety issues with wandering residents and people loitering in the lobby. The dialysis center and rehab therapists were excellent - therapy helped - and some staff were compassionate; security and parking were adequate. New ownership, fresh paint and nicer furniture are positive, but the culture, med-dispensing system and nursing care still need real improvement. I pulled my loved one out quickly.

    Summary review

Overall sentiment across the reviews is mixed but leans toward concern: multiple reviewers acknowledge tangible recent improvements (new ownership, uniforms, fresh paint, furniture, and a more visible security presence), plus clear strengths such as an on-site dialysis center, active therapists, and some compassionate staff and rehab outcomes. However, a recurring and significant theme is highly inconsistent care quality. Several accounts describe serious neglect (patients found soaked or bed-wet, not being changed or bathed appropriately), missed or incorrect medications, ignored emergency call buttons, and delayed responses that in at least one case led to ER transport. These safety and basic-care failures are among the strongest negative signals in the reviews and are reported across multiple, independent summaries.

Staff performance appears highly variable. Many reviews praise individual therapists and certain nurses for being attentive, delivering daily therapy, ensuring medicines, and creating a family-like atmosphere. Conversely, other reviews describe staff who are inattentive, unprofessional, bickering or socializing at the nurses' station while residents go unattended, and in some reports even allegedly unwilling to call 911. High turnover, use of cheap labor, and a reportedly poor management culture are cited as root causes of uneven staff competency and morale. Several reviewers filed formal complaints and one mentioned potential legal action, reflecting the seriousness of some incidents and the frustration families feel with administration responses.

Facility and amenities present a split picture. On the positive side, reviewers note renovations (fresh paint, new furniture), presence of amenities like a church, beauty salon, and shop, scheduled group activities (exercise, singing), strict security at times, and parking availability. The dialysis center and rehabilitation services receive clear praise from multiple reviewers, with daily therapy described as a standout. On the negative side, the building is described as aging or dated despite cosmetic updates; two-person rooms with only curtains compromise privacy; there are repeated complaints about unpleasant odors, soiled conditions, and even incidents of residents sitting in the rain or wandering from the front entrance. Some reviewers questioned how deep or lasting the renovations and staffing changes truly are.

Dining and clinical operations show specific, repeatable issues. Meal quality is frequently described as bland with limited choices, and multiple reviewers said food sometimes arrives cold or stale, although dietitians are available for individualized needs. Medication management is a consistent concern: reports include missed or incorrect meds, outdated dispensing methods, and an overall sense of unreliability in pharmaceutical care. While some families said nurses ensured meds and nighttime safety checks were in place, the counterexamples raise significant safety flags.

Population and service mix influence perceptions: several reviews note a largely Medicaid-resident population and the facility's reputation for taking very difficult or high-acuity cases. Some reviewers see this as a sign the facility handles complex needs (and point to the dialysis center and rehab successes), while others feel it contributes to strain on staff and resources and correlates with neglectful incidents.

Management and governance emerge as central themes. There are credible reports of improvements after ownership change—uniforms, security, and cosmetic upgrades—but also persistent skepticism about whether changes address deeper cultural and operational problems. Multiple reviewers reported unhelpful administration when trying to resolve serious issues. Complaints and reports of potential litigation indicate that some families believe problems are systemic rather than isolated.

Bottom line: Deanwood Rehabilitation And Wellness Center shows clear areas of genuine strength—particularly in rehabilitation therapy services, an on-site dialysis center, and some evidence of recent investment and compassionate staff members. However, the volume and severity of negative reports about nursing care, hygiene, medication management, emergency responsiveness, and safety incidents weigh heavily. These are not merely minor service complaints but recurring, serious concerns that have led some families to remove loved ones quickly. Prospective residents and families should weigh the demonstrated rehab and dialysis strengths against the documented risks, ask detailed questions about current staffing levels, turnover, medication protocols, incident reporting, and what concrete steps management has taken to ensure consistent, reliable nursing care and resident safety. Frequent family involvement and close monitoring are advised while the effectiveness and durability of the reported improvements remain unclear.
