    Clean facility but inconsistent management

I had a mixed experience. The facility and grounds are clean, the nurses and therapy staff were caring and helped my mom regain strength, with good food and activities - but communication and management were inconsistent: phone calls went unanswered, POA/billing issues, some staff unresponsive or rude, and lapses in medical orders caused serious problems for residents. I'd recommend this place for short-term rehab; for long-term or high-acuity care, proceed with caution.

    Summary review

Overall sentiment across the reviews is highly mixed, with many families praising the facility’s physical environment, therapy outcomes, and compassionate staff, while a substantial minority report serious lapses in communication, clinical follow-up, and basic care. The facility receives consistent positive mention for its well-maintained, one-story building, attractive grounds and central courtyard, and newly remodeled independent living apartments. Residents and families often note cleanliness, comfortable rooms, available special equipment, and an active calendar of activities and outings. Meals and snacks are commonly described as good, and transportation services (including bus trips to the senior center and appointments) are viewed positively.

Clinical care and staffing generate the strongest divergence in experiences. Several reviews highlight excellent nursing care, effective rehabilitation therapy (with patients regaining function, speech, and mobility), and compassionate palliative or end-of-life support where staff went "above and beyond." Multiple accounts specifically commend attentive, kind, and knowledgeable nurses and a helpful admissions director who facilitated placements. These positive reports emphasize timely check-ins, encouragement from staff, and rehabilitation aimed at returning residents home.

Conversely, other reviews describe troubling clinical and operational issues. Recurring concerns include poor communication—phones unanswered for hours, calls returned slowly or not at all, and families being unable to reach or speak to residents before critical events. Some families report that physicians did not order necessary tests (for example urine tests), resulting in repeated UTIs and hospitalizations. Serious allegations include catheter failures, residents left in urine, inadequate hygiene, weight loss from small meal portions, and even claims that medical deterioration or death was linked to neglect or insufficient care. Several reviews assert that nurses did their best but were hamstrung by lack of physician orders or by being overworked, suggesting systemic staffing or management challenges rather than solely individual caregiver failures.

Management and administrative practices also draw polarized feedback. While the admissions director is singled out positively in some reports, other reviewers describe poor customer service, denial of refunds for costly respite care, and a perceived prioritization of revenue (including complaints related to VA payments and high daily rates). Families express frustration with POA restrictions and administrative responsiveness. Reports of rude or unwelcoming front-desk or administrative staff, combined with accounts of low pay and overworked personnel, indicate morale and resource issues that may contribute to inconsistent care.

A notable pattern is the variability of experiences: many reviewers had overwhelmingly positive interactions and impressive rehab outcomes, while others recount severe shortcomings that would lead them to strongly advise against the facility. Several reviews suggest that nurses and aides often provide compassionate, capable care but may be limited by physician decisions, understaffing, or administrative constraints. This variability points to the importance of timing, individual staff members, and specific units or shifts in determining care quality.

For prospective residents and families, the reviews suggest concrete areas to investigate during a tour and intake: ask about physician coverage and how medical orders/tests are handled, inquire about catheter and incontinence protocols, review staffing ratios and turnover, verify infection-control and hygiene practices, sample meal portions and menu variety, clarify respite-care costs and refund policies, and confirm communication protocols for families and POAs. Also, request recent outcomes for rehabilitation patients, examples of successful discharges, and specifics on palliative care approaches.

In summary, Rim Country Health & Rehabilitation displays many strengths—clean, attractive facilities, robust therapy services, meaningful activities, and numerous accounts of dedicated, compassionate staff and successful care. However, there are repeated and serious negative reports regarding communication failures, physician-order issues, hygiene and catheter incidents, perceived neglect, and administrative/customer-service problems. The reviews recommend careful, specific due diligence by families considering placement, because individual experiences can vary widely and critical issues reported by some reviewers warrant direct questioning and verification before admission.

The nursing home part has 109 beds where registered nurses, therapists, and caregivers provide care around the clock, whether it's long-term help, ongoing rehabilitation, wound care, or therapy like physical, occupational, or speech, and they even help people coming out of hospitals and those who need hospice or palliative care or just need a little time to recover before heading home, with services that include things like dialysis, cardiac care, and behavioral health help for seniors with depression or anxiety. You've got room choices like semi-private rooms or apartment-style units, and the place is designed for both privacy and community, since there're shared spaces like a dining hall for restaurant-style meals that care about nutrition, a fitness room, small library, computer center, beauty salon, wellness center, and outdoor courtyard space for enjoying fresh air or walking the dog, because they allow pets, and activities like bridge, games, and various programs run throughout the week, so there's always something to do for folks who can and want to join in. Rim Country also arranges rides to the doctor and gives complimentary in-state medical transport for admissions, while keeping a 24-hour emergency alert system and having staff nearby at all times, plus they provide adult day health care, respite services for families that need a break, placement help, and discharge planning, with extra support and counseling for both residents and families, and a resident council meets to have their say about things.
