    Compassionate, professional care; beautiful facility

I moved my mom here after a hospital stay and have been very pleased. The facility is beautiful and spotless, staff are warm and attentive, nursing and therapy teams accelerated her recovery, meals are tasty, and activities keep residents engaged. Admissions and social work made the transition easy and kept us informed - I'd recommend Country Hills to families looking for compassionate, professional care.

    Summary of reviews

Country Hills Post Acute presents a polarized picture: many families and patients describe high-quality, attentive clinical care—particularly from nursing and rehabilitation teams—while others report systemic operational weaknesses that affect overall reliability. Positive accounts emphasize effective short-term rehabilitation, motivated therapists, and nursing staff who facilitate return to daily living activities. Admissions and certain social-work staff frequently receive praise for easing transitions from hospital to facility. The facility's renovated interiors, windowed rooms, and active programming contribute to a welcoming, family-friendly atmosphere for many residents.

At the same time, recurring patterns suggest variability in experience depending on unit, shift, or specific personnel. Reviewers describe inconsistent staffing levels, which appear to contribute to delays in attending to call lights and inconsistent assistance. Families cited gaps in communication and coordination between nursing, social services, and front-desk staff, producing unclear discharge plans or difficulty reaching key personnel. Several accounts pointed to inconsistent infection-control and quarantine practices in specific units, as well as sanitation and pest-control concerns that undermine confidence in environmental safety.

Dining and nutrition are described unevenly: when kitchen leadership is present, meals and service are praised, but reviewers also describe inconsistent food quality and failures to accommodate allergies or preferences on other shifts. Activities and social programming are repeatedly highlighted as strengths; the facility organizes a range of events that many residents enjoy and that support a family-like community culture.

Management and operational issues are a frequent source of dissatisfaction. Specific concerns include limited transparency about billing and shared-room costs, parking and access-management tensions, and at least one alleged privacy/communication breach. While some staff members and leaders are singled out for exemplary effort and responsiveness, others are described as unmotivated or difficult to engage, indicating variability in supervisory effectiveness. There are also isolated but serious allegations—including claims related to belongings management and unauthorized room access—that families called to the attention of regulators.

For prospective residents and families, Country Hills may offer strong rehabilitation services and a warm social environment when staffing and departmental coordination are functioning well. However, due diligence is advisable: ask for current staffing ratios, observe cleanliness and pest-control measures in relevant units, confirm allergy and dietary accommodations, request examples of recent discharge plans and interdisciplinary meeting notes, and verify privacy and billing policies. Visiting during different shifts and speaking directly with the therapy team, nursing leadership, and admissions/social-work can help determine whether the facility’s strengths align with an individual’s needs and whether operational inconsistencies have been addressed.

    About Country Hills Post Acute

Country Hills Post Acute sits under the PACS Group, Inc., which owns several care centers in the San Diego area and helps local teams focus on taking care of residents. The building looks nice and feels clean, with a large gym full of good music and fresh smells. On-site staff and licensed nurses stay available day and night, always ready to help. The place keeps to post-acute and skilled nursing care and has a strong focus on patient well-being and a quality life, and if you need to see a doctor, you can set up a free consultation with one of their general physicians, like Dr. Sanjay Rajpara, Dr. Elaine Gertser, or Dr. Bernard Michlin, and you'll also see visits from specialists in internal medicine, some with over 40 years of experience. There's a full list of services for many medical needs-like abdominal pain, kidney infections, DVT, bronchitis, upper and lower respiratory issues, urinary incontinence, sciatica, and even help after hip replacements or surgeries, and if you need to check in for administrative physicals, you can do that too.

Counseling sits offered here, run by psychiatrists and psychoanalysts, and if you need memory care, Alpine View Lodge is a part of their service network, handling special needs in that area. You'll see licensed therapists in the gym helping people with rehab, therapy, and IV therapy management, and the gym itself stands out since it's big, state-of-the-art, and always seems to have something going on, from group classes to exercises led by experts. The facility opens every day from 9 in the morning to 9 at night, which makes visits easier for families, and it's got common areas and spacious courtyards so everyone can relax or spend time together. You can check out the facility in advance with virtual photo tours and videos online.

They host organized and sometimes creative volunteer programs, and you'll see them set up different classes, activities, and events to keep people involved and active, which seems to help give everyone a sense of purpose and a well-rounded day. The whole approach focuses on working together with families, staff, doctors, and volunteers to give each resident a chance to heal and feel strong as possible, and you'll notice there's a steady partnership with both the health care team and the community. Safety and comfort stay high on their list, and though the place stays busy, you can tell that care for each patient comes first. The nice lobby and outside spaces give people a place to rest or get some air, and the counselors and medical team stick around to make sure every person gets what they need, whether it's help with medical issues, regular physicals, or a bit of extra support after a tough surgery.
