    Compassionate professional excellent rehabilitation care

I had a very positive experience - the staff were genuinely caring, professional, and responsive, with attentive nurses, receptionists, and excellent PT/OT/SLP therapists who went above and beyond for my mother. The rehab space and activities program are great, the facility is very clean, bright, and welcoming, and I appreciated the compassionate communication from Administrator Brayden, NP Harman Gill, and social worker Alina. This caring, well-run place changed my perception and I'd recommend it.

    Summary of reviews

Reviews of Covenant Post Acute present a mixed but coherent picture: many families and visitors describe attentive, skilled clinical teamsโ€”particularly physical, occupational, and speech therapistsโ€”who deliver rehabilitation-focused care and measurable functional improvements. CNAs, reception staff, and some social-work and case-management personnel are frequently characterized as compassionate and responsive, and several families praised individual clinicians and administrators for clear communication and advocacy on behalf of patients.

At the same time, a number of reviews identify operational weaknesses that affect clinical consistency. The primary concern is variability in responsiveness to resident needs: delayed call-button response, inconsistent attention to personal-care tasks such as bathing, and uneven adherence to clinical maintenance tasks (including catheter and infection-related management). These issues appear to cluster with reports of staffing instability and morale/resource constraints, suggesting that outcomes can differ noticeably by shift or unit.

Dining and housekeeping comments are similarly mixed. Several visitors praised the facility's cleanliness, bright common areas, and pleasant atmosphere, while others mentioned inconsistent meal quality and isolated sanitation or supply shortfalls. The activities program is a clear strength in many accountsโ€”reviewers commonly note a robust calendar, active staff participation, and opportunities for resident engagement that families find meaningful.

Facility and management impressions are polarized. Some reviewers highlight improvements under new management and describe timely, helpful administrator interactions; others report difficulty reaching leadership, delayed external transfer coordination, and gaps in administrative follow-through. A recurring operational concern is the co-mingling of residents with cognitive impairment and those receiving short-term rehabilitation, which some families felt contributed to distress or distraction on certain units.

Overall pattern: the facility demonstrates clear capacity to provide high-quality, rehab-focused care with compassionate staff and strong activity offerings, but experiences notable variability in consistency of care and operational reliability. Prospective residents and families would benefit from asking about staffing levels by shift, protocols for catheter and infection management, orientation and meal schedules, separation of care populations, and procedures for emergency transfers to understand how current practices match their clinical and safety needs.

    About Covenant Post Acute

Covenant Post Acute operates day and night, providing 24/7 nursing care for seniors who need help after surgery, illness, or injury, and the place has a long-term focus on recovery and wellness with a range of healthcare services like rehabilitation, skilled nursing, and specialized therapy, and everyone who stays in one of the 121 certified beds gets an individual care plan, regular meetings for them and their families, and daily housekeeping and laundry. The team there includes registered nurses, licensed therapists, a medical director, nurse practitioners, nutritionists, nurse assistants, vocational nurses, and an admissions crew, all used to helping with things like wound care, stroke rehab, diabetes, cardiac care, ostomy and catheter care, IV therapy, and activities to help with balance, memory, and emotional health, plus there are programs for fall prevention, incontinence care, joint replacement, and help with daily living tasks if needed. The place has a big rehab gym, some semi-private rehab suites, and large, comfortable common areas where people can build relationships-something that's important to everyone involved-and there's Wi-Fi, daily meals planned for special needs, and emergency call buttons in every room for safety. Covenant Post Acute is owned by Aht Holdings, LLC, managed since April 2021 by Aht Holdings, LLC and Lucille Epperson, and organized as a for-profit Limited Liability Company with indirect owners Pakhdavor, LLC, Jacob Unger, and Julianne Williams. Covenant has a nurse turnover rate of 45.5%, which is more than the state's average, and offers 3.80 nurse hours per resident a day, also below the state average. Past inspections found some issues, like a citation for not fully honoring residents' rights to a safe and clean home (F0584), some equipment safety concerns (F0908), and infection control procedures (F0880), all without any actual harm but with a possibility for issues, and in total, there are 54 deficiencies with 6 linked to infection control, but the staff keeps the place looking clean and tries to keep things home-like. Covenant's Medicare page gives more public info about their services and their record. People at Covenant Post Acute get recovery care with a strong focus on making them feel better, along with programs for health, financial, mental, and emotional support, career growth for staff, and efforts to keep things as close to a family environment as possible.
