    Outstanding short-term rehab compassionate staff

I stayed at Team Pacific Gardens for short-term rehab and was very pleased. The nurses, CNAs and therapists were kind, attentive and skilled - PT/OT helped my recovery. The building was spotless and odor-free, meals were good, and there were plenty of activities and festive touches. Admissions and discharge staff (Denise, Jocelyn, Ashley) were responsive and personalized my care; I felt respected and well cared for and would recommend this facility.

    Summary of reviews

The body of reviews for Pacific Gardens Nursing and Rehabilitation Center presents a strongly mixed picture. A large number of comments praise frontline caregivers — CNAs and nurses — and the therapy teams (PT/OT/Speech) for compassionate, skilled, and outcome-focused care. Several reviewers described meaningful functional gains, successful discharges home, and helpful case-management and discharge-planning staff who coordinated follow-up services. Family-facing staff in admissions and social work received repeated positive mention for being informative and accommodating. The facility also receives positive notes for organized activities, bilingual support, festive programming, wound-care competency, and in many accounts clean, well-presented rooms with reliable bedside telephones and accessible call lights.

At the same time, recurring operational weaknesses appear across the dataset. Many reviews describe variability in staff conduct and professionalism between shifts and individuals, with specific concerns about responsiveness to assistance requests and call lights. Staffing variability and periods when CNAs and nurses appeared stretched were linked to delays in routine care, nighttime supervision gaps, and cases where families perceived care coordination or monitoring to be insufficient. Several entries identify inconsistent medication handling or documentation and broader gaps in clinical communication, including situations where family notification or consent-related paperwork was unclear.

Dining and activities show a split: reviewers frequently appreciate the availability of activities, special events, and that some meals and guest dining are well done; however, there are repeated comments about inconsistent meal quality, repetitive menus, and limited meal planning for residents with specific dietary needs. Facility upkeep and environmental concerns are similarly mixed. Many reviewers noted clean rooms, fresh linens, and general housekeeping diligence, while others raised sanitation and odor concerns in certain common areas or bathrooms, as well as stained equipment and limited housekeeping in peripheral spaces. Maintenance and equipment reliability—nonfunctioning phones, call-button issues, small TVs, and non-adjustable beds—were noted as operational shortcomings that affect resident comfort and safety.

Administrative and safety themes also recur. There are allegations of theft and billing irregularities that some families felt warranted investigation; such claims, alongside a handful of comments about unclear billing and delayed or missing referrals, point to the need for stronger property controls and administrative transparency. Families also reported variable communication from management around transfers, hospital notifications, and end-of-life documentation; a few accounts described serious disagreements about informed-consent processes. Nighttime supervision, fall-prevention practices, and supervision during transfers emerged as areas where families wanted clearer protocols and more consistent staffing.

In sum, Pacific Gardens appears capable of delivering high-quality, rehabilitative care when staffing and coordination align, with notable strengths in therapy services, individual caregiver dedication, and active social-work support. However, the facility also demonstrates operational inconsistencies — in staffing levels, responsiveness, cleanliness in select areas, meal consistency, security of personal property, and administrative transparency — that prospective residents and families should evaluate during a tour. Recommended pre-admission questions include: current staffing ratios by shift, call-light response protocols, property-security measures for valuables, medication- and consent-documentation procedures, how therapy intensity and progress are documented, and examples of how management handles billing questions and family communication. These checks can help determine whether the facility’s strengths match a given resident’s needs and priorities.
