    Good short-term rehab, risky long-term

I had mixed experiences here. The building is very clean, staff are generally friendly, hardworking and great at rehab/physical therapy, activities and meal service - I was grateful for several compassionate caregivers. However I also saw serious communication and management breakdowns: missed/late meds, rude or neglectful aides, safety lapses, premature discharges and even incidents that required ER visits. If your loved one is alert and can advocate for themselves, this can be a good short-term rehab; I would not trust it for total, long-term dependent care without constant oversight.

    Summary review

Overall sentiment: Reviews of Ramona Rehabilitation & Post Acute Care Center are strongly mixed, with many detailed accounts praising the therapy programs, individual caregivers, cleanliness, and activity offerings, while a significant number of reviews raise serious concerns about medication management, neglect, communication, and administrative practices. The dominant positive theme is high-quality rehabilitation and many compassionate staff members; the dominant negative theme is inconsistent clinical and managerial performance that in some cases compromised resident safety.

Care quality and clinical themes: Multiple reviewers emphasize exceptional rehabilitation services — physical and occupational therapists repeatedly receive high praise for being skilled, attentive, and personalized in their approach. Several families report measurable functional gains and clinicians who go above and beyond (even working off-shifts) to help patients improve. Nursing and CNA performance is frequently described positively as well, with numerous staff members named and lauded for kindness, responsiveness, and attention to patients. However, juxtaposed with these positive accounts are numerous, serious allegations of clinical lapses: medication errors (delayed, missing, or not given), inconsistent glucose/insulin management, falsified documentation, and long delays in tending to soiled residents. There are multiple reports of neglectful or abusive treatment, including residents left in waste, verbal abuse, and physical safety incidents. These conflicting patterns suggest variability in care quality that may depend on shift, specific personnel, or unit.

Staffing, communication, and management: Reviews frequently praise individual staff members and teams, but many comments criticize management-level issues. Common complaints include poor communication between nursing, rehab, and physicians; unreturned or slow responses to calls/messages; and short-notice or abrupt discharge decisions without adequate supplies, medications, or equipment. Several reviews describe situations where administration appeared to cover mistakes, minimize family concerns, or fail to follow through. Staffing levels are a repeated concern: reviewers report overworked nurses and CNAs, rushed care, and sometimes long waits for call lights to be answered. These workforce challenges appear linked to many of the negative care experiences.

Facility, cleanliness, and environment: The facility is consistently described as clean, well-decorated in many areas, and organized by many reviewers. Positive notes include fresh-smelling spaces, well-maintained grounds and engaging common areas. Yet some reviewers report unpleasant odors (urine, dirty diapers, mold), hot or poorly ventilated rooms, and an older building that would benefit from renovation. Parking limitations at the front and the facility's age are practical concerns mentioned by several families.

Safety and discharge practices: There are multiple reports of unsafe discharge practices — patients discharged without walkers, medications, or proper transfer planning — with at least one post-discharge fall and subsequent emergency care referenced. These incidents, coupled with allegations of theft (wallets, walkers) and falsified records, raise red flags about safety oversight, documentation integrity, and transitions of care. Families who had coordinated, well-communicated discharges describe positive transitions, but the inconsistency is a notable pattern.

Dining, activities, and resident life: Activity programming, social events, and holiday celebrations are frequently praised; reviewers note daily activities, bingo, music events, and an active planning coordinator. Meal quality is mixed: some reviewers call food excellent and note staff assist with feeding, while others criticize meal portioning, missed lunches, and unmet special-diet needs. The facility is repeatedly recognized as creating a warm social environment for many residents.

Notable positive patterns: When a resident's primary need is skilled rehabilitation, many families strongly recommend Ramona — citing therapist expertise, attentive nursing collaboration with rehab, and rapid functional improvements. Several reviewers specifically commend hospice care, long-term custodial transitions handled well, and staff who advocate for residents' needs.

Notable negative patterns: Serious and recurring concerns include medication mishandling, delayed or absent personal care, inadequate documentation, erratic management responses, and reports of neglect or abuse. These issues are often tied to specific shifts, personnel, or administrative failures and have, in some accounts, led to ER visits or harm after discharge.

Recommendations for prospective families: Based on the review patterns, this center may be an excellent option for patients primarily seeking intensive, therapist-led rehabilitation and for those who can advocate for themselves or have active family involvement. However, families of patients requiring total or high-dependency care should exercise caution: perform an in-person tour, ask targeted questions about medication administration protocols, staffing ratios by shift, incident reporting and theft prevention measures, discharge planning procedures, and language support. Verify nursing leadership availability, request to meet the primary therapists and assigned CNAs, and confirm receipt of medications and necessary equipment before any discharge.

Bottom line: Ramona Rehabilitation & Post Acute Care Center has many strengths — notably an effective rehab program, numerous compassionate staff members, strong cleanliness and activity programming — but it also displays recurring systemic issues around medication safety, communication, staffing, and discharge processes that have led to serious negative experiences for some residents. Quality appears variable by unit, shift, and individual caregivers, so careful vetting and active family involvement strongly influence the likelihood of a positive stay.

About Ramona Rehabilitation & Post Acute Care Center

Ramona Rehabilitation & Post Acute Care Center sits at 485 W. Johnston Avenue in Hemet, California, and it's a for-profit nursing home and post-acute care facility with 104 certified beds and an average of 94 residents each day, which means there's a steady crowd but you won't usually find it packed full. Owners Robin Leland, Stanford Leland, and Heidi Vickers manage the place, and while the staff does include some dedicated people like Violeta CNA, who's been described as patient and friendly, there have been issues reported too. The facility has received a total of 32 deficiencies in inspection reports, with six of those relating to infection control, and others dealing with not reporting suspected abuse or neglect quickly enough, along with some concerns about following treatment orders and matching residents' wishes and goals. The nurse turnover rate here is 47.7%, which is higher than the California state average, so you may see new faces from time to time, and the nurse staffing hours per resident per day comes in a bit under the state average at 4.44 hours. On the bright side, Ramona Rehabilitation & Post Acute Care Center has some notable amenities to help make the place feel comfortable for residents, and there's a range of therapy and special programs meant for people recovering after hospital stays, so some will get more therapy depending on what they need. The care team includes specialized staff and, while not every aspect runs perfectly, plenty of staff members do aim to keep folks comfortable and supported. The facility uses hospital equipment and furniture to make things a bit easier, and people can check out the place online through a virtual tour, which is nice for getting a feel for the rooms and set-up without going in person. For anyone who needs help with long-term care, family caregiving, or guidance about insurance, Ramona offers resources to support those needs and stays open for admissions any time of day or night. While the facility has earned a 5-star rating from the Centers for Medicare and Medicaid Services and a 4.0 average from 30 online reviews, the deficiencies show that there are areas that could use some improvement, so it's a good idea to weigh both the positives and negatives when considering a stay here.
