    Attentive compassionate care, clean facility

I had a very positive experience - the staff (nurses, CNAs and therapists) were caring, knowledgeable and often went above and beyond to make my mom feel at home. The facility is clean and welcoming, with a pleasant courtyard and a day room for family visits. Communication from administration and social work was responsive and supportive, and therapy helped with recovery. I'd recommend this place for attentive, compassionate post-acute care.

    Summary of reviews

The review set for Haven Post Acute presents a highly mixed picture. Positive comments emphasize compassionate bedside care, capable wound management, and effective therapy-based rehabilitation that helped some residents return home. Families frequently praised CNAs, select nurses, and therapy staff for being attentive, encouraging, and recovery-focused. The facility's activities program, communal spaces (courtyard and sunroom), and meal offerings were also cited as meaningful contributors to resident quality of life in many accounts.

At the same time, a recurring cluster of operational concerns appears across reviews. Several families described sanitation concerns and intermittent odor control in portions of the building. Staffing levels and supervision were noted as inconsistent, which reviewers linked to longer response times for assistance, delayed pain medication, missed treatments or pickups, and variable adherence to documented care plans. These issues contributed to reported gaps in medication and pain-management processes and raised concerns about fall-prevention and safe transfer practices.

Communication and administration were another pattern. While some families experienced clear, responsive coordination and positive admission/discharge planning, others reported poor follow-through from leadership and social services, difficulty recovering personal belongings, and limited weekend/holiday coverage. There are also allegations of property loss and related financial handling problems that families found distressing. Emergency-response and hospital-transfer coordination were described as delayed in several accounts, and reviewers associated those delays with avoidable stress during critical events.

Dining and activities receive generally favorable mentions: meals were described as balanced and supportive of recovery by many, and activity staff were repeatedly appreciated for engagement and individualized encouragement. Facilities impressions are mixedโ€”some reviewers described clean, well-kept areas and helpful reception staff, while others flagged sanitation and pest-control concerns in specific units. Overall, this body of feedback suggests a facility with clear strengths in hands-on nursing, wound care, and therapy services, but with operational and management inconsistencies that affect safety, property management, communication, and reliability of care. Prospective residents and families should weigh the positive clinical and rehabilitation capabilities against the variability in housekeeping, staffing coverage, and administrative responsiveness, and ask targeted questions about fall-prevention protocols, medication administration procedures, property safeguards, weekend staffing, and emergency-transfer policies during any tour or intake conversation.

    About Haven Post Acute

Haven Post Acute sits in San Bernardino at 1311 East Date Street, and it's a 99-bed skilled nursing facility that takes care of seniors with various medical and rehabilitation needs, offering both long-term and post-acute care, and Medicare and Medicaid coverage makes it easier for families who need financial assistance, so you'll find rates for private rooms ranging from $6,000 to $8,000 and semi-private rooms between $5,000 to $7,000 per month. Managerial control and ownership come from Bryan Boehrer and Richard Martin since May 2021, with direct ownership listed under BVHC, LLC, so there's a steady group in charge. Though Haven has a warm, welcoming family environment and features like a centralized courtyard, game room, dining room, and lobby with receptionist, inspection reports show 29 deficiencies, including ones for pharmacy services with issues about labeling and locking up drugs (F0761), not providing quite enough food and fluids (F0692), problems with meeting food preparation standards (F0812), plus one infection-related deficiency, which the reports say could lead to more than minimal harm if left unchecked, though there was no actual harm found at the time, and nurse turnover here is 56.4%, which sits above the state's average, while nurse hours per resident reach 4.01, a little below the state average. Residents experience skilled nursing care twenty-four hours a day, with friendly professionals, help for daily activities like bathing, dining, and dressing, wound care, medication support, and diabetic and dementia-related care, with rehabilitation options including individualized therapy, pain management, and exercises for range of motion. Activities lean toward social and creative, offering things like arts and crafts, games, a fitness center, educational and health programs, and you'll see a strong focus on keeping things lively, plus the property has cable TV, washers and dryers, Wi-Fi, maintenance, kitchens or kitchenettes, and safety features like sprinklers. There's transportation, guest parking, and the full support of an activities team and social services staff who help residents and families handle requests and communication, and they do regular monitoring to make sure residents maintain healthy nutrition, which includes tailored meal provisions with supplements and recorded checks for height and weight every week or month. Haven Post Acute helps families look at different options for Medicare and Medicaid, welcomes tours and family visits by appointment, and always tries to promote a peaceful, home-like setting, though inspection records and staffing numbers are worth noting if you're comparing options.
