    Unsafe neglectful dirty poorly managed

I would never recommend this place. In my experience the staff were neglectful, often rude or unreachable (hung up on callers), and slow to respond-placing residents at real safety risk with missed appointments, delayed hospital transfers, falls, and inadequate therapy. The building felt dirty and rundown (urine smell, overflowing toilets, missing paint, hot crowded halls, non-working TVs), and staff attention was inconsistent despite a few kind, competent people (Cheryl Moore and some PT/nursing/social work staff). Overall: unsafe, poorly managed, and not a place I'd trust for a loved one.

    About Brookside Care Center

Brookside Care Center has 99 certified beds and sits in Stockton, California, where folks needing skilled nursing, surgery, or rehabilitation can find a range of services. Licensed nursing staff are on site, and they provide care around the clock, with an average of 3.87 nurse hours per resident per day, although the nurse turnover rate is at 31.1%, which is something to think about if stability is important. The facility belongs to AJC Healthcare and operates as a for-profit limited liability company. Safety and supervision get special attention here, with staff focusing on keeping residents safe and comfortable, though inspection reports have flagged some infection-related issues and also noted deficiencies in accident hazard precautions and certain resident rights and safety matters. Management details aren't really available, but the staff tries to offer patient-centered care, treating each person's needs as unique. Brookside does offer advanced surgical technology and skilled care, with specialized surgical services, like orthopedic and general surgery, plus pain management and GI procedures. Rehabilitation therapy is available, with speech, occupational, and physical therapy programs, and there's a social services team ready to help families during stays or transitions. Activities help keep folks engaged and the atmosphere a bit more positive, and general counseling is also part of what's offered. Brookside aims to make each resident as comfortable and well-cared for as possible, though it's clear from inspection reports that there's still room for improvement in some areas.
