Wagner Heights Nursing & Rehabilitation Center sits in Stockton, California, and has 152 beds for residents needing different levels of care, so you'll see both short-term rehab services and long-term stays here. Nurses stay on the job 24 hours a day, and the staff includes dietitians who handle meal plans, therapists for rehab, social workers to help with daily problems, and nursing assistants who do much of the hands-on work. You'll find treatments for wound care, stroke and cardiac recovery, pain management, diabetes, tracheostomy, and even hospice care if needed, and each service gets attention from specialists trained in that field. The center also offers pulmonary rehab, respiratory programs, orthopedic fracture recovery, and programs like Homeward Bound that use newer technology and proven therapy practices to help people return home faster. Physical, speech, and occupational therapists work up to seven days a week, which helps people improve their speaking, walking, or using their hands for daily living, and there's special support for those who need stroke, cardiac, or orthopedic rehab. Amenities include home-cooked meals in a big dining area, outdoor spots to relax, group activities, and community outings when folks are able. Residents can join daily activities that keep them social and engaged. The center has special programs for things like Congestive Heart Failure, Continence Improvement, Contracture Management, and Dysphagia, and offers services like IV and enteral therapy, TPN/PPN, ophthalmology, ostomy care, and post-acute surgical care, as well as fall management to help people stay safe. Wagner Heights is run by Covenant Care Inc., which manages over 40 healthcare centers in the country, and they accept new residents at any time of day, working to help each person reach as much independence as possible.

Neglectful, unsafe, untrustworthy care facility
I would not recommend this place. My experience included neglect, unresponsive and underqualified staff, poor communication, unsafe conditions (untreated wounds/bedsores, urine smell, roach sightings), lost/stolen belongings, and terrible discharge planning that forced private transport and led to hospital transfers. Management was unstable and defensive, and care felt profit-driven and expensive. A few nurses, CNAs and therapists were genuinely kind and the rehab/therapy area and courtyard were bright spots, but overall I would not trust them with a loved one.
