About Crescent Oaks - Memory Care

Crescent Oaks - Memory Care sits in a quiet Sunnyvale neighborhood and specializes in helping adults with Alzheimer's and other forms of dementia, and they call their specialized support Aviator Care Plan, which is all about making sure each person gets attention to their own unique needs, and when you walk into the newly renovated two-story building, you see it was made for safety and comfort, with secure doors and special memory care areas. There are private and semi-private suites, some are studios, and everyone's encouraged to bring their favorite things from home to decorate and keep that feeling of familiarity, and you'll notice their house is a real community, so folks are always invited to join in a variety of activities-indoors and outside on the Four Seasons outdoor room and open patio-which are part of their structured program for socializing, friendships, and keeping minds active with cognitive exercises every day. Staff is on duty 24 hours, trained in memory care, always ready to help with everything-bathing, eating, medication, getting dressed-while giving residents the space to do what they can on their own, and in between, people gather for meals made from scratch three times a day, with snacks available, and special diets are considered. Amenities include in-house beauty and barber services, TV cable, Internet, phone, and complete laundry and house cleaning every day, and there's also transportation for doctor visits and help arranging visits from podiatrists, dentists, and therapists if needed. The team at Crescent Oaks offers a continuum of care, so someone can get assisted living, nursing care, or independent living as their health changes, and with regular health checks and daily observation, families know their loved one's doing alright. The community focuses on supporting memory and dignity, helping residents keep up hobbies or discover new ones, and everyone gets offered seven days of activities, entertainment, and fitness programs. The environment is safe and pleasant, designed for both comfort and engagement, and the staff keeps learning new ways to make life better for residents with dementia or memory impairments. Facilities have updated infrastructure and amenities, and Crescent Oaks has a history of memory care, with communities also in Menlo Park and Montara, California.
