    Excellent care with minor issues

I've had an excellent experience - the staff are caring, professional, and responsive, nursing and rehab (therapy pool/physical therapy) are top-notch, meals are homemade and customizable, and rooms feel private, warm, and safe. Residents stay engaged with activities, outings, and a strong resident-to-staff ratio, and COVID precautions were handled well. I did notice occasional meal delays and a few distracted aides/management lapses, but overall my loved one was very well cared for and I highly recommend this place.

    Summary review

Overall sentiment across the reviews is strongly positive, with the most consistent praise focused on staff quality, cleanliness, and a comfortable, home-like environment. Reviewers repeatedly describe the staff as compassionate, attentive, professional, and present—nurses and CNAs are frequently singled out as knowledgeable and caring. Multiple comments emphasize a strong resident-to-staff ratio and effective teamwork, which contributes to feelings of safety and trust when placing loved ones in Cottonwood Rehabilitation and Healthcare Center. Several reviews specifically call the facility "capable hands" for both rehabilitation and longer-term care needs.

Care quality and rehabilitation are commonly commended. Reviews report reliable physical therapy services and tangible rehabilitation progress for many residents; the presence of a therapy pool and tailored physical therapy programs is highlighted as a meaningful asset. That said, there are reports that rehabilitation results are not uniformly successful for every resident—some reviews say rehab produced good results for certain individuals but imply inconsistency. Overall the facility is positioned as a strong option for skilled nursing and for patients needing post-acute rehab, with repeatedly noted examples of staff going above and beyond.

Facilities and atmosphere receive frequent positive mention. The building and grounds are described as clean, organized, aesthetically pleasing, and welcoming. Reviewers note warm, cozy décor that contributes to a home-like feeling, along with private and spacious rooms and a private courtyard. Practical conveniences such as a smooth checking and checkout process, dog-friendly visits, and a family-friendly environment also appear across reviews. Cleanliness, organization, and general upkeep are consistent themes that reinforce the positive impressions of the physical environment.

Dining and activities are strengths for many reviewers. Homemade, fresh, and customizable meals are repeatedly praised—food is described as delicious and tailored to resident preferences. At the same time, a recurring operational concern is occasional delays in meal service; several reviews mention timely service issues even as they praise the food quality. Activity programming (crafts, outings, and other engagement opportunities) is mentioned positively and supports the facility's home-like and social atmosphere.

Management and communication are described in largely favorable terms by most reviewers: proactive management communication, responsiveness to questions and concerns, and strong leadership are cited as contributors to the facility’s positive culture and teamwork. However, an important pattern to note is that a small but notable minority of reviews raise concerns about management-staff relations—allegations that management treats staff poorly, that management lies, or that nurses are reluctant to engage due to management issues. These negative reports are outnumbered by positive comments about leadership, but they represent a recurrent, contrasting theme that prospective families may want to probe further during a visit.

There are a few service-focused personnel concerns documented in the reviews. While many reviewers praise CNAs and nursing staff, some observations note aides talking among themselves or appearing distracted, and a few residents or families reported feeling invisible at times. These comments are less frequent than the positive reports but are relevant because they speak to variability in day-to-day resident experience. In short, the dominant narrative is one of warm, professional, and attentive care, with intermittent lapses noted primarily around staff attentiveness and meal timing.

In conclusion, Cottonwood Rehabilitation and Healthcare Center consistently earns high marks for staff commitment, cleanliness, atmosphere, dining quality, and rehabilitation resources. The most prominent strengths are the caring and professional staff, the home-like environment, and solid rehab offerings (including a therapy pool). Areas to watch are occasional meal service delays, isolated reports of staff distraction or residents feeling overlooked, and a small set of concerning reports about management-staff relations. Prospective residents and families should feel confident in Cottonwood’s overall quality but may wish to ask targeted questions on a tour about staffing continuity, CNA engagement, meal schedules, and how management addresses staff concerns to ensure these occasional issues are understood and mitigated.

About Cottonwood Rehabilitation and Healthcare Center

Cottonwood Rehabilitation and Healthcare Center sits in Durango, Colorado, just east of the Mercy Regional Medical Center complex, and folks know it for providing short-term rehabilitation and long-term skilled nursing care in a small, hometown setting with 40 certified beds. The staff uses specialized therapy services and state-of-the-art technology to help people with different needs, whether they're recovering from the hospital or dealing with conditions that make them dependent on nursing. They offer programs in cardiac and orthopedic rehabilitation, pain and diabetes management, wound healing, pulmonary care, and more, always aiming to help their guests feel comfortable and safe each day. There's a focus on collaboration with residents and families; they try to treat everyone like family, and people can see that through their approach to care and community connection.

Cottonwood serves as a VA contracted facility, so veterans can use their benefits here, and the team works to help guests get back home through a full-service rehab program. Skilled nursing staff provide expert care, and the building has amenities meant to create a warm and welcoming feel. They also provide specialty services for behavioral health along with regular intermediate and long-term care. Nurse turnover is 48.6%, which is lower than the state's average, and reported nurse hours per resident are above the state average, too. You'll hear that management has been led by Refoel Gottlieb since September 2022, and the owner structure is set up as a for-profit partnership, managed through Centennial Colorado Holdco, and the current administrator is Christy Bradley. The facility has had some documented deficiencies, including quality of life and care issues related to behavioral health and infection control, but these were rated as having no actual harm, though there's always a chance for more if they're not watched. Staff and leadership do their work within the standards required by their membership in the Colorado Health Care Association District VI.
