    Clean facility with compassionate staff

I toured and placed my mother here and feel very reassured - the facility is clean, residents seemed happy, and the nurses and CNAs consistently went above and beyond. Admissions and admin were professional and supportive (Chassidy was especially patient and helpful and answered all our questions, including Medicaid assistance). Care is compassionate and competent, rehab progress was rapid, and I'm grateful for the positive experience.

    Summary of reviews

Shores Nursing and Rehab Center elicits strongly mixed impressions. Positive accounts emphasize compassionate caregivers, effective short-term rehabilitation outcomes, and supportive assistance with Medicaid and placement logistics. Several families described individual staff members who were reassuring and attentive, and some residents appeared satisfied with daily care. The admissions and marketing team is frequently described as professional and helpful during intake and tours.

At the same time, a pattern of operational concerns appears across reviews. Staffing levels and available direct-care hours are uneven, and families describe delays or gaps in communication about changes in condition and care plans. Management responsiveness is a recurring issue: some accounts describe difficulty obtaining timely administrative follow-up. There are also descriptions suggesting inconsistency in clinical practice, including wound and infection management and coordination for higher-level care transitions.

Facility- and environment-related issues are another prominent theme. While portions of the building are described as clean and orderly, other areas have sanitation and odor-control concerns, deferred maintenance, and older equipment. Pest-control concerns and limitations of the physical plant were noted; these highlight variability in environmental upkeep across the campus. Reported problems with laundry, clothing storage, and personal belongings point to operational process gaps that affect family confidence.

Care culture and communication present a split picture. Positive interactions and rapid rehab progress are balanced by accounts of low staff morale, inconsistent visibility of staff on units, and instances where front-desk or on-shift coverage was limited. Families also raised concerns about transfer and hospitalization coordination, including refusals or delays to escalate care when families expected transfer. A small number of serious allegations—including claims of deceptive staff conduct and instances prompting external involvement—underscore the need for clearer transparency and complaint-resolution pathways.

For prospective residents and families: Shores offers demonstrable strengths in rehabilitative therapy and has staff members capable of compassionate, reassuring care. However, decision-makers should probe operational reliability: ask for current staffing ratios, examples of recent infection-prevention and maintenance initiatives, the facility’s transfer/hospitalization protocols, and how personal belongings and laundry are handled. Visiting during different shifts and speaking directly with nursing leadership can help assess whether the facility’s positive aspects are consistent with day-to-day operations.

About Shores Nursing and Rehab Center

Shores Nursing and Rehab Center, also known as Cross Shores Care Center and associated with the Bridge at Bay St. Joe and Signature Healthcare, sits in Port Saint Joe, FL and has room for 120 certified beds with an average of about 82 residents a day, offering skilled nursing and a full range of rehabilitation services, and you'll find things like long-term care, memory care, active adult living, independent living, assisted living, adult day services, home care, certified home health care, and hospice care, plus programs for special groups like veterans. Residents have personalized care plans and programs set up just for their needs, along with features that work to improve well-being and mobility, while amenities focus on comfort during recovery. The place takes Medicare and Medicaid, and it's got a staff made up of nursing professionals and a team aimed at giving compassionate support to folks who need help with medical issues and daily living activities. Shores is managed by Jack Harrison and Daniel Thorngren since December 2024 and carries direct ownership by 220 Ninth St Port St Joe Holdco LLC, with other owners like New Horizon Healthcare LLC and Sarah Becher and Eliyahu Mirlis involved too, and though the team works to create a comfortable setting, state inspections have flagged the center for 31 deficiencies including problems like not having enough staff on some shifts, missing regular nurse aide training, and not always following rules for infection control, so it's had serious quality issues but isn't currently under special federal attention. Shores offers a lot of options, but folks needing constant or routine medical attention might want to know staff don't always check on patients regularly or monitor vital signs as closely as some other places do, though people staying here have unique care setups and a mix of services that aim to fit many individual needs.
