    Caring staff but serious concerns

I appreciate the family-like atmosphere-CNAs and rehab staff do an excellent job keeping residents active and building strong relationships with families and the community, and leadership feels positive and caring. However, I've also experienced rude staff, one alarming emergency birth where delivery care was inadequate and the midwife had to step in, and I was billed for services I never received.

    Summary review

Overall sentiment in the reviews is predominantly positive, emphasizing strong clinical and interpersonal strengths at Clearwater Health and Rehabilitation of Cascadia. Multiple reviews praise the caregiving staff—particularly CNAs—for delivering attentive, compassionate care and building strong, trusting relationships with residents and families. The facility’s rehabilitation program is highlighted as effective and proactive, with staff described as skilled at getting and keeping residents moving within their scope of abilities. Reviewers frequently note a family-like atmosphere and good community connections, which contributes to a sense of warmth and social support for residents.

Care quality and clinical services receive high marks in several reviews. CNAs are repeatedly singled out for doing a “great job” caring for seniors, and rehabilitation staff are described as "on their game," suggesting an organized, competent therapy program focused on functional mobility. One review summarizes the overall care as “amazing,” indicating that some family members view the facility as delivering consistently strong clinical attention and meaningful relationship-building between staff and residents.

At the same time, there are serious negative remarks that contrast with the generally positive tone. One review alleges rude staff behavior and describes a very specific and concerning incident during a delivery/emergency birth in which the reviewer claims care was inadequate and a midwife ultimately had to catch the baby. That same review also asserts billing for services that were not performed. These are significant complaints: the clinical allegation (an emergency incident) and the administrative allegation (incorrect billing) warrant follow-up. Because these criticisms are severe but appear in a single review, they register as notable outliers that should prompt verification rather than broad conclusions.

Staff and management impressions are mixed but lean positive. Several reviewers praise friendly staff, good leadership, and a positive administrator, indicating effective management and a supportive culture at the leadership level. However, the presence of at least one report of rude staff and the billing/clinical incident point to possible inconsistencies in training, supervision, or accountability that could allow isolated lapses to occur. This pattern suggests that while leadership and many staff perform well, there may be variability among employees or incidents that fall outside the facility’s usual standards.

Facilities, dining, and many day-to-day operational details are not well described in the provided summaries. The strongest operational details concern rehabilitation and activity-oriented care that promotes mobility. The "family life atmosphere" comment suggests a social environment that supports resident engagement, but there is no direct information on dining quality, physical plant condition, or routine activities beyond rehab and social relationships.

In summary, the dominant themes are high-quality, compassionate caregiving (especially from CNAs), an effective rehabilitation program, and strong leadership that fosters positive relationships with residents and families. These strengths create a family-like environment valued by reviewers. Counterbalancing these positives are isolated but serious negative allegations—rude staff behavior, a claimed mishandled emergency birth, and disputed billing—that should be investigated by the facility. Overall, the reviews indicate a generally well-regarded facility with specific areas (staff consistency, clinical incident response, and billing accuracy) where management should confirm practices and address any problems to ensure those outlier incidents do not recur.

About Clearwater Health and Rehabilitation of Cascadia

Clearwater Health and Rehabilitation of Cascadia sits at 1204 Shriver Rd in Orofino, ID 83544, and you'll find that the staff take care to put together treatment plans that fit each person's needs, whether someone's staying short-term to recover or for the long-term with more help needed every day, and the facility runs with 24-hour skilled nursing care along with patient-centered rehabilitation programs. The place has 60 certified beds and usually about 39 residents each day, with rooms that can be private or semi-private, which gives people some choices about their space. There's a range of services here, including nursing care, short-term rehabilitation, transitional care, medication management, specialized help for those with dementia or Alzheimer's, and palliative or hospice care when needed, plus residents have support from social and recreational activities to help keep up community life and stay engaged. Clearwater's team includes nurses and therapists trained to provide physical, occupational, and speech therapies, and they focus a lot on helping each person work toward their own health goals. The facility's pretty close to the hospital, making it easier to get to in emergencies or after surgeries or illnesses. People can come in for tours to meet the staff and see how the place feels before making a decision. There's management from Cascadia Healthcare, and since January 2017, Warren Bodily has held managerial control, with direct ownership from Cascadia Idaho Operations LLC and indirect stakes held by Cascadia Healthcare LLC, Warren Bodily, Owen Hammond, Nathan Hosac, and Matthew Smith. The building has to follow federal rules about infection prevention and resident care, and while the staff work hard for patient safety, recent inspections, like one on April 17, 2025, reported 17 deficiency citations, with some about infection control, resident assessment, care planning, and a few involving nutrition and protection of residents' property, so some areas have room for improvement. Nurse turnover stands at 36.4%, with nursing care hours averaging 3.29 per resident per day, which can affect consistency of care at times. Clearwater's a for-profit limited liability company, and while they serve mainly in English, some staff may speak other languages. They're not currently taking new patients but can let folks know when a spot opens up. The environment aims to be supportive and welcoming, with comfort-focused amenities and a code of conduct to protect residents' privacy and rights. The staff try to help each person live as well as possible day to day, adapting care plans as situations change.
