    Mostly good care with concerns

I moved my mother here two years ago and overall it's been a very good experience: nice rooms, bright windows, garden/open courtyard, hair salon, pets brought in, movie nights and Bingo, and a large, competent rehab team that helped with decisions. The nurse aides and CNAs are caring, one full nurse oversees care, and the two-sided setup (nursing + assisted living) met her needs and is close to home; food is adequate (institutional). My main concerns: care dipped badly at one point, some staff seemed unwilling to manage certain residents and even threatened hospitalization, and the Alzheimer's unit may have been misrepresented.

    Summary review

Overall impression: The reviews present a generally positive but mixed picture of Discovery Rehabilitation And Living. Multiple reviewers emphasize strong, compassionate frontline caregivers, robust rehabilitation services, and a well-maintained physical environment with pleasant outdoor spaces. However, at least one serious negative account raises concerns about inconsistent care for some residents and potential management or communication problems. The dominant themes are solid hands-on care and good amenities tempered by occasional reports of poor responsiveness and a disputed memory-care representation.

Care quality and staff: The most frequently mentioned strengths are the nursing aides, CNAs, and a perception of competent, caring staff. Reviewers specifically note that nurse aides are "wonderful" and CNAs are helpful, and one reviewer noted that a full nurse oversees care, which suggests centralized clinical oversight. The facility's rehabilitation team is described as large and active, and staff are credited with being helpful during rehab decision-making. Those positives indicate reliable support for short-term rehab and day-to-day assistance. Contrastingly, one review described "awful care," including an instance in which staff were reportedly unwilling to handle a particular patient and a family felt threatened with hospitalization. That single, serious negative account stands out against the otherwise positive staff comments and suggests there may be variability in how challenging cases are handled or in staff training/availability for higher-need residents.

Facilities, amenities, and activities: Physical plant and amenities receive consistent praise. Reviews mention nice, well-maintained rooms (including two-person rooms with adequate space), a garden and open courtyard, attractive windows, and on-site hairdressing. Pet visits are allowed, which several reviewers appreciated. Social programming is present and noted by residents—movie nights and Bingo are specifically called out—indicating an active social calendar. These elements point to a facility that invests in resident quality of life and communal spaces, supporting both short-term rehabilitation stays and longer-term assisted living needs.

Dining: Food is commonly described as "adequate" but "institutional." While reviewers do not focus intensely on meals, the repeated characterization of food as typical of institutional settings suggests it meets basic needs without standing out as a strength. Prospective residents who prioritize culinary variety or restaurant-quality dining may find this an area to investigate further.

Management, consistency, and red flags: The reviews show a pattern of generally competent care but with at least one notable negative report involving alleged misrepresentation of the Alzheimer’s unit and a family-feeling that staff were unwilling to manage a particular patient and threatened hospitalization. These are substantive concerns: misrepresentation of specialized memory-care services and aggressive escalation to hospital transfer, if accurate, are serious issues for families to consider. The mixed accounts imply that quality and approachability may vary by staff, shift, or specific resident needs. There is not enough information to determine frequency, but the presence of a strong positive majority alongside a severe negative outlier suggests inconsistent experiences rather than uniformly poor care.

Overall conclusion: Discovery Rehabilitation And Living appears to offer solid rehabilitation services, attentive CNAs and nurse aides, and pleasant facilities with social programming and outdoor spaces. Most reviewers report positive, helpful staff and well-kept surroundings. However, prospective residents and families should be aware of potential variability in handling complex or behavioral cases and should probe specifics about the Alzheimer’s/memory-care offerings, staffing levels and training for higher-need residents, the facility's policies on hospitalization and incident escalation, roommate arrangements for two-person rooms, and sample menus. Given the mix of generally favorable comments and one serious adverse account, an in-person tour, conversations with clinical leadership, and checking recent regulatory/inspection records would help verify that the facility's services align with an individual’s needs.

    About Discovery Rehabilitation And Living

Discovery Rehabilitation And Living sits in Salmon, Idaho, and offers a mix of assisted living, memory care, independent living, skilled nursing, and rehabilitation services, so you'll find support whether a person needs a little help with daily activities or thorough nursing care around the clock, which can be reassuring for families, and even though folks say the place has a 45-bed capacity, others note there's a medium-sized 24-bed wing, but the main thing to know is, this facility isn't too large for the staff to know their residents by name but is big enough to provide things like a beauty salon, barber services, cable TV, Wi-Fi, garden spaces, community dining rooms, walking paths, and well-kept grounds, making day-to-day life comfortable. Discovery makes sure to keep the rooms clean and the beds made with laundry and housekeeping, and the accommodations come furnished in both private and semi-private options, which helps if someone's recovering from a hospital stay or needs rehab for a while. The facility operates with a 24-hour call system and supervisions for peace of mind, and you'll see nurses and caregivers step in with things like medication management, diabetes diet support, help with bathing, dressing, transfers, and support during tough times, plus memory care and mental wellness programs for those who need it. The focus at Discovery is meeting a person's unique needs, so staff, families, and healthcare providers work together to come up with care plans whether a resident's there for a short or long stay, and being part of the Ensign Services network helps keep care reliable and affordable. Residents can join movie nights, daily activities, social events, or go on outings near the mountains, parks, places of worship, and restaurants, which can break up the routine and keep people active. The therapy staff uses both modern and classic rehabilitation tools, hoping residents regain their strength and move as independently as possible, and the grounds offer a safe place to walk or just relax. Meals are nutritious, the surroundings are pleasant, and the community's designed so folks don't feel crowded or lost in the shuffle-more like they're in a caring, supportive environment with plenty of chances to enjoy their day and receive good, honest care.
