    Summary review

Overall sentiment: The reviews for Brickyard Healthcare - Brentwood Care Center are predominantly positive, with a strong and recurring emphasis on compassionate staff, effective leadership, and a clean, welcoming physical environment. Many reviewers describe the staff as caring, family-like, and responsive; leadership and management receive repeated praise for being supportive and moving the facility in a positive direction. The facility’s appearance—newer updates, pleasant décor, and good housekeeping—appears consistently to contribute to a homelike atmosphere that residents and families appreciate.

Care quality and clinical themes: A large portion of reviewers report high-quality, person-centered clinical care, timely responsiveness to urgent issues, excellent therapy services, and positive clinical outcomes. Several reviews specifically highlight attentive CNAs, well-mannered nurses, and an effective therapy team. At the same time, there are several serious negative reports that cannot be ignored. These include medication administration problems (doses given late or incorrect medications), reports that some residents did not see a physician, and comments that therapy was provided without a proper documented evaluation or diagnosis in certain instances. While the majority of comments praise the care teams and clinical outcomes, these negative accounts introduce concerns about consistency in clinical oversight and medication safety practices.

Staff, leadership and culture: Many reviewers single out an "amazing" leadership team and endorse both floor staff and department heads. There is a strong theme of teamwork, staff who treat residents like family, and staff who are genuinely invested in the residents’ well-being. Activities such as celebrations, special events, and individualized attention underscore the facility’s person-centered approach. Contrastingly, a minority of reviews report troubling staff behavior—yelling or using profanity toward residents—and allegations that management retaliated against family members or attempted to limit family contact. These reports point to possible incidents of misconduct or poor conflict management that appear to be isolated but serious. The overall culture appears positive for most families, but the presence of these allegations indicates variability in staff behavior and managerial response.

Facilities, activities and daily life: The facility’s physical environment is repeatedly praised: clean, freshly updated, well-maintained, and inviting. Housekeeping is often called out as exceptional. Residents and families frequently mention meaningful activities and events (petting zoo, Easter egg hunts, parades, costume parties, birthday celebrations) that enhance quality of life and community engagement. There are also mentions of small conveniences like in-house hairdressing and socially engaging programming. A few operational issues appear despite the overall cleanliness: isolated reports of urinals not being emptied and laundry problems (lost or ruined clothing) indicate that occasional lapses in routine housekeeping/household management do occur.

Communication, transparency and patterns of concern: Communication is described variably—many reviewers commend the facility for timely, professional communication and excellent customer service, including responsiveness during admissions and to urgent needs. Conversely, other reviewers describe communication as inconsistent or improving only at times, and a few allege lack of transparency from management. The most significant negative patterns are medication errors, allegations of improper therapy practice without documented evaluation, physician access concerns, and claims of staff misconduct and management retaliation. These issues, while not the dominant tone of the reviews, are serious in nature and suggest areas where the facility should ensure robust policies, clear documentation, and transparent incident reporting.

Net assessment and implications: Taken together, the reviews portray Brickyard Healthcare - Brentwood Care Center as a well-regarded facility with many strengths: compassionate and dedicated staff, strong leadership that has driven improvements, a clean and updated environment, rich activity programming, and generally positive clinical and therapy experiences. However, the presence of multiple serious concerns—even if reported by a minority of reviewers—warrants attention. Medication safety, staff conduct training and accountability, documented clinical evaluations for therapy, consistency in physician oversight, and reliable housekeeping/laundry processes are the primary areas of risk highlighted by the reviews. For prospective residents and families, the overall picture is one of a facility with many real strengths and a strong reputation among many families, but with some notable outlier experiences that should prompt direct questions during tours and admissions (for example, asking about medication administration protocols, incident reporting, staffing patterns, and how family concerns are escalated and resolved). Ensuring these topics are addressed can help reconcile the overwhelmingly positive themes with the important but less frequent negative reports.

About Brickyard Healthcare - Brentwood Care Center

Brickyard Healthcare - Brentwood Care Center has 114 certified beds and offers skilled nursing, short-term rehabilitation, and long-term care for people who need extra help between hospital and home or ongoing daily support, and you'll find they offer memory care services with a secure setting and outdoor area for residents with Alzheimer's or other kinds of dementia, with staff trained to handle different memory and cognitive needs. They run therapy programs including occupational, physical, and speech therapy, and there's a rehab gym with parallel bars and other equipment to help with healing after surgery or illness, plus their therapy options are available both in-house and as outpatient care for those who don't live there but need support. Rooms come with cable or satellite TV, adjustable beds, emergency call systems, and views, and you'll see that they try to make their suites comfortable for residents whether they're staying short-term or long-term.

Families will notice a full range of health services like pain management, restorative care, IV therapy, stroke care, dialysis care, diabetes support, and pulmonary therapy, along with basic dental, eye, and audiology visits, and they also include pharmacy support and regular doctor visits. Their daily activities keep people busy with arts and crafts, church services, recreational programs, snacks, and transportation for outings, plus there's free Wi-Fi, a barber and beauty shop, private meals, and laundry or housekeeping. VA Contract services mean veterans can use some of their benefits here, and you'll see the facility also offers Respite Care for families needing a break from caregiving.

They're known for providing individualized care and try to live up to their promise of focusing on tenderness and comfort, but like most places, they have some challenges; for instance, state reports show 18 documented deficiencies, including failing to give the proper care for urinary and bowel needs (F0695), not always managing care for incontinence, catheters, or urinary tract infections as required (F0690), and issues with pharmacy storage and labeling (F0761). There are also three deficiencies connected to infection-related violations, so paying attention to infection control should be a priority, and their nursing staff spends about 3.51 hours per resident each day, which is just below the state average of 3.7 hours, though their nurse turnover rate is a bit better than the state as a whole.

Ownership comes from Hendricks County Hospital, and management has stayed with Ggnsc Evansville Brentwood LLC since 2012, with Shelley Brown as the Executive Director. The team includes physicians, psychiatrists, and psychoanalysts who're responsible for decisions about clinical care. Brentwood Care Center also houses children under special circumstances alongside its adult residents, provides general counseling, and offers personal care services. Their unique mix of skilled nursing, therapy, memory care, and range of amenities makes them one option in the community for anyone seeking support with complex or ongoing medical needs.
