    Kind staff, poor medical oversight

I had a mixed experience here. The staff were mostly kind and supportive, the facility felt home-like and clean, and the meals and therapy programs were excellent. However, I also saw understaffing, poor leadership communication, equipment in hallways, and worrying lapses in medical care, so I'd recommend with caution and only after confirming staffing and care protocols.

    Summary review

The reviews for Brickyard Healthcare - Woodlands Care Center present a strongly mixed picture in which many families and residents report high-quality, compassionate care while a smaller but highly serious subset report dangerous lapses and poor facility management. Positive comments focus repeatedly on the staff — many reviewers use words like friendly, kind, compassionate, and supportive — and praise specific aspects of care such as attentive therapists, robust rehab activities, and accommodations for swallowing issues and soft-food diets. Several reviewers emphasize a home-like atmosphere, cleanliness, and that residents appear happy and well cared for. Dining receives consistent positive mention, including specific praise for meals (notably a pot roast) and staff members who ensure meals are healthy and enjoyable (Becky is singled out by name). These positive reports also include multiple family endorsements recommending the facility for long-term stays and trusting the staff to care for relatives as their own.

Counterbalancing these endorsements are serious and specific negative reports that raise safety and management concerns. The most alarming complaints allege negligence and a failure to provide prescribed medical care, including one allegation centered on a night nurse (identified as Troy) and a subsequent patient death. Reviewers describe failures to follow hospital-directed treatments, delayed care, denial of access to outside providers (such as a podiatrist), and leadership that does not follow up or respond to family concerns. These kinds of complaints suggest potential breakdowns in clinical oversight, communication, and adherence to care plans — issues that directly affect resident safety.

There is also inconsistency regarding the facility environment and staffing. Some reviewers report the center as clean, well-kept, and having a welcoming, home-like feel; others describe it as filthy, understaffed, and neglected, with equipment left in hallways and tiny, cramped rooms. Multiple reviews note the facility is busy, and a few explicitly state understaffing or that care did not meet expectations during shorter admissions (for example, being discharged within 48 hours with unsatisfactory care). This pattern — strong positive experiences for many alongside a smaller number of very negative, potentially dangerous incidents — indicates variability in performance that may depend on unit, shift (several complaints specifically reference night shift), or specific staff on duty.

On balance, the dominant themes are polarization and inconsistency: many families are very satisfied and praise the staff, therapy services, and dining, but a minority of reviews describe severe failures in medical care, responsiveness, and cleanliness. For prospective residents and families this suggests a need for targeted due diligence: ask about staffing levels by shift (especially night coverage), medication and care plan handoff procedures, how hospital orders are implemented, policies for outside provider access, incident reporting and follow-up practices, and recent inspection or complaint histories. When visiting, observe mealtime, therapy sessions, hallway clutter, room sizes, and staff-resident interactions; request to speak with leadership about the specific concerns raised in reviews. The reviews show the facility can provide excellent, compassionate care, but the reported serious safety lapses and management communication failures warrant careful inquiry before making placement decisions.

About Brickyard Healthcare - Woodlands Care Center

Brickyard Healthcare - Woodlands Care Center sits at 4088 Frame Rd in Newburgh, IN, and has 120 certified beds, though about 105 residents stay there on an average day, so you do get a mix of long-term and short-term folks coming in and out, and the place has been managed by Ggnsc Newburgh LLC since September 2012, with Executive Director Maribeth Donaldson leading the team, bringing over three decades of skilled nursing experience, and she's been with Brickyard Healthcare for 20 years. The care center offers a wide range of services, including skilled nursing, short-term rehab, long-term care, memory care set up for people with Alzheimer's or dementia, outpatient therapy, dialysis, and VA contract care, and the building is also set up for independent living, assisted living, adult day services, home health care, non-medical home care, and hospice care, though details about apartment style or assisted living features aren't heavily listed. The place's daily staffing boils down to about 3.36 nurse hours per resident, but nurse turnover runs a bit high at 39.2%, and the place has been through 17 reported inspection deficiencies, with a few related to infection standards-such as citation F0880 for infection prevention programs and one about pharmacy storage and labeling, though these didn't cause actual harm but did have the potential for issues, even if only a few people were directly affected. For residents, there are private suites, phone and Wi-Fi, and TV options, plus laundry and housekeeping, and the staff handle a variety of clinical tasks such as IV therapy, pain management, cognitive training, diabetes care, and more serious rehabilitation for folks after strokes, along with regular doctor, dental, eye, and audiology visits. The community provides church services, daily activities, arts and crafts, nutritional snacks, and has barber and beauty shops, along with regular transportation services, and folks can visit in family lounges or the secure outdoor area, especially in the memory care section where extra safety measures and trained staff step in for residents who might wander or need a little extra care because of dementia. There's access for wheelchairs, but not much is told about the general building layout or features like parking, entrances, floors, or exterior space, and while the care staff handles a lot of health and personal needs, there's not much information on luxury amenities or unique rental setups, but you do get a full range of therapy and nursing for the folks who need it, and the facility stays connected with doctors and hospitals like St. Mary's nearby to make sure residents have medical support when they need it.
