Hazelwood Center sits on 52 acres in Louisville, Kentucky, and has helped people with intellectual and developmental disabilities since 1971, with the facility itself opening in 1972 and now operated by New Vista since 2018. The place offers assisted living and intermediate care facility services, focusing on both medical and daily living needs, and three community ICF homes called Del Maria, Windsong, and Meadows give residents a personalized, homelike setting. Hazelwood Center covers everything from medication management and daily living help-like bathing, dressing, and transfers-to nutritious meals with restaurant-style dining, allergy-sensitive choices, diabetes diets, and a chef who makes sure there are meals all day. The rooms have private bathrooms, cable TV, Wi-Fi, phones, kitchenettes, and air conditioning, and residents can use areas like the library, game room, theater, activity room, garden, spa, sauna, walking paths, fitness room, and wellness space, all while enjoying landscaped outdoor spaces.
The Hazelwood Specialty Clinic at the center takes care of complex health needs, giving residents and the community access to primary care, dentistry, psychiatry, neurology, ophthalmology, audiology, and behavioral health, along with therapy services like physical, occupational, and speech therapy, plus wound care, nutritional guidance, and psychological and behavioral management. An interdisciplinary team sets up each person's care plan, making sure it fits their needs and goals, and staff provide support around the clock every day. The center focuses on personal growth and independence, offering more than eleven enrichment classes each day, along with music, art, and fitness activities, and chances to join community baseball, faith groups, and Special Olympics events. There are work and volunteer opportunities on campus and in the area, helping people learn job skills and connect with others, and programs help with money, meal planning, shopping, and other daily skills.
Hazelwood Center partners with advocacy groups, universities, and medical experts, so they keep up with best practices and ongoing learning for staff, offering continuing education, internships, and practicums, and there's strong help for benefits coordination and health management, too. Residents can get support with housekeeping, laundry, move-in coordination, and concierge services, with transportation and parking available for outings and mobility. The place offers a 24-hour call system, supervised care, on-site restrooms, wheelchair accessibility, air conditioning, and customer parking, so residents' comfort and safety get a lot of attention. People with intellectual and developmental disabilities can expect care that balances dignity, independence, and wellbeing, with programs that meet a wide range of medical, psychological, behavioral, and personal needs, led by Ms. Kelly Boggs as Facility Director. The administrative offices and clinics operate Monday through Friday, and the campus supports both professional and personal growth for residents, staff, and students, making Hazelwood Center a long-standing part of the Louisville community for those needing specialized, compassionate support.
