    Professional attentive staff, strong rehab

I'm very pleased with the care my mom received. The staff were professional, attentive and genuinely caring-Robin and PT James were especially helpful-therapists, nurses and CNAs coordinated well and rehab results were strong. Rooms were clean, meals good, and activities kept residents engaged. Communication was timely and staff went above and beyond; I would recommend this facility.

    Summary of reviews

Overall impression: Reviewers portray Hermitage Care & Rehabilitation Center as a facility with strong hands-on caregiving and a rehabilitation focus, paired with a homelike, friendly atmosphere. Many families praised individual staff members, CNAs, nurses and therapists for compassionate, attentive care and for producing measurable rehab gains after surgery or illness. The facility appears to perform well when delivering therapy-driven short-term stays and has several private rooms dedicated to rehabilitation.

Care and staff: Care quality is described as generally good to excellent when staff are engaged: nurses, CNAs and therapists are often characterized as caring, attentive and supportive. Families highlighted effective communication on care plans and timely responses from maintenance and some clinical staff. However, multiple comments indicate inconsistent staff conduct and responsiveness at times; reviewers described occasions when tone, timeliness of assistance, or hand-hygiene adherence did not meet expectations. These accounts suggest variability in day-to-day staff performance and a need for more consistent practice across shifts.

Rehabilitation and clinical services: The facility receives consistent praise for its rehabilitation program and therapy teams (physical, occupational and speech therapy). Reported outcomes include noticeable functional improvement and strong therapy engagement. Clinical coordination between therapy and nursing was frequently described as effective, which supports positive short-term rehabilitation results.

Dining and activities: Dining experiences are mixed. Several reviewers complimented fresh, well-seasoned meals and accommodating service, while others described poor or inconsistent food quality. The activity program is a clear strength: organized events such as bingo, happy hour, outdoor time and room-based options for less-mobile residents contribute to resident engagement and morale.

Facilities and environment: Rooms and public areas are generally described as clean and well-maintained, with quick maintenance response and no pervasive sanitation issues cited. At the same time, the building is older and some furnishings appear dated. Reviewers also noted that the physical layout is not purpose-built for memory-care residents, which may limit suitability for those with significant cognitive impairments. Semi-private rooms introduce standard privacy and roommate-compatibility challenges.

Management, communication and operational patterns: Positive accounts of strong leadership and coordinated care coexist with concerns about management stability and communication, particularly during transfers and critical events. Reviewers described lapses in personal-item tracking during transfers and uneven family communication during infection events and end-of-life transitions. There were also references to disciplinary actions and staffing changes, suggesting periods of leadership or staffing turnover. Together, these indicate opportunities to strengthen transfer protocols, family communication practices, and consistency of oversight.

Notable patterns and recommendations: Prospective residents and families can expect a therapy-oriented environment with many compassionate frontline staff and an active life-enrichment program. Important considerations include the facility's older infrastructure, potential variability in meal quality, and episodic staffing or communication gaps. Families who prioritize strong rehab services, a warm atmosphere, and frequent activities may find the Hermitage appealing; those with advanced memory-care needs or who require consistent private accommodations should assess unit layout, roommate options and communication protocols before placement.

About Hermitage Care & Rehabilitation Center

Hermitage Care & Rehabilitation Center sits on West Parrish Avenue in Owensboro, Kentucky, in a single-story building that has room for up to 92 residents, and many folks like the way it feels homey and friendly. The center accepts Medicaid as payment, and the staff provides 24/7 care, working hard to help with each person's needs whether day or night. There are private rooms and several floor plans, so people have choices about where to live within the building. The courtyard outside is pleasant, and residents can relax or just enjoy being outdoors when the weather's nice. The center provides meals in the main dining room, and they hold community activities, social events, and special programs during holidays, which help the days pass with something to look forward to every now and then.

Special attention goes into personal care, medication management, and memory care, with services for folks who are living with dementia. Wound care and rehabilitation are strong parts of what the Hermitage team handles too, and they run various therapies-physical, occupational, and speech therapy all happen right in the building, so residents don't need to go elsewhere. The staff has a reputation for being warm and compassionate, which gives families some peace of mind. Each care plan is tailored to the resident, and people can get short-term rehabilitation after a hospital stay or long-term skilled nursing care if that's what's needed. There's a nursing and rehabilitation center of excellence here, and the focus is on making life comfortable and treating everyone with dignity. The facility pays attention to spiritual needs and quality of life through special programming that tries to meet everyone where they are, including those from different backgrounds and cultures. Group outings and life enrichment programs make sure residents stay engaged with the world outside, and all the support is there to help people live as well as possible, whether they're here for a little while or a long time. The doors are open every day, with regular office hours during the week, and the staff keeps connected through care navigation and integrated care networks that help with overall satisfaction. The environment really tries to bring a sense of comfort and home to residents, focusing most on dignity, clear communication, and everyday living.
