    Summary review

Overall sentiment in the reviews is strongly positive but punctuated by several serious concerns. Many reviewers emphasize the facility’s recent renovations and attractive, clean appearance; multiple comments describe a warm, home-like atmosphere with friendly, caring staff who treat residents like family. High staffing levels, attentive nurses and CNAs, effective housekeeping, and strong medication adherence are recurrent strengths. Families frequently praised the admissions team for being responsive and helpful with paperwork and insurance, and they noted rapid patient progress under the center’s care. The presence of a new memory care unit was highlighted repeatedly as a positive development for residents with Alzheimer’s or dementia.

Care quality and staff culture are central themes. Most reviewers report compassionate, resident-focused care: courteous nurses and CNAs, therapists and restorative aides who support functional recovery, and staff that go “above and beyond.” Therapy services—especially physical therapy—receive praise from many families for contributing to recovery and progress, and the facility is described as suitable for both short-term rehabilitation and long-term stays. Activities programming and social opportunities are noted as robust, supporting social engagement and friendships among residents. Technological conveniences such as video conferencing for family visits and visible security features (facial recognition, monitored doors) were mentioned as reassuring and modern additions.

Facility operations and amenities are generally well-regarded. Multiple comments highlight the remodeled interior, attractive exterior, and excellent housekeeping. The new memory care unit and the availability of respite care or short-term stays were viewed as important offerings that meet diverse family needs. Admissions staff and leadership were singled out as helpful and efficient, easing transitions, assisting with insurance/paperwork, and making the move-in process smoother for families. Several reviewers also described strong customer service and an environment that feels like family and friends rather than an institutional setting.

However, despite the largely positive picture, there are several serious negative issues raised by reviewers that merit attention. A handful of reviews allege rough handling by staff and cite a bruise that prompted concerns of possible abuse or neglect; one review explicitly advised families to keep loved ones away. Related to dementia care, some reviewers said residents with cognitive impairment were not given adequate time to adjust and were assessed and transferred out rather than supported in place. A few reviewers simply noted the facility felt “unsafe” or flagged safety concerns without providing details. These reports are relatively less frequent than the positive comments but are significant due to their severity and the vulnerability of the population served.

Other areas for improvement are more operational: several reviewers suggested nutrition could be improved, and a few requested expanded physical therapy availability despite overall praise for the therapy team. These recurring suggestions indicate opportunities to enhance resident experience—particularly in dining and therapy scheduling/volume. The mixed nature of reviews—overwhelmingly many positive experiences contrasted with a small number of serious allegations—suggests variability in individual experiences or possible lapses in consistency of care.

Recommendations based on the review themes: continue to maintain high staffing, strong housekeeping, and medication adherence; preserve and further promote the warm, family-oriented culture that many families value; maintain and leverage security and tech features that support safety and family contact. Concurrently, investigate and address allegations of rough handling and any reported safety incidents through clear incident reporting, staff retraining in safe transfer and dementia care techniques, and transparent communication with families. Review policies and practices around transitions for dementia residents to ensure adequate adjustment time and person-centered supports before decisions to transfer. Finally, evaluate dining services and therapy capacity to address the repeated suggestions for nutrition improvements and increased therapy access. Overall, the facility demonstrates many strengths in environment, staffing, and rehabilitation, but should address the serious safety and dementia-care concerns to ensure consistent, high-quality care for all residents.

About Princeton Health & Rehab Center

Princeton Health & Rehab Center Under New Management is a Skilled Nursing Facility with 104 beds, caring for about 78 residents, and folks will notice right away that the place got renovations in 2020, which made it look brighter and more cheerful than before, though the building really does try hard to give a home-like feeling, and it's managed these days by the Pennyrile Area Development District, a regional planning group, so some changes by the new management are still ongoing. This facility serves older adults who need different types of care, including skilled nursing, help with memory problems, personal assistance for getting dressed, bathing, or moving around, and there's a focus on people who need support for things like diabetes, special diets, wound care, or even more complex needs such as dialysis.

There are 12-16 hours of nursing care each day and a call system set up for 24-hour help, which is there for emergencies or when someone needs extra support, and nurse staffing averages almost three hours per resident each day, which helps with both medical and daily needs, including medication management and transfers. Medical services at the center include physical, occupational, and speech therapy, along with help for pain, wounds, and post-hospital rehabilitation programs meant to help folks get their strength back and, in some cases, return home. Safety comes through emergency alert systems, door alarms, and wander guards for residents who need supervision, with quick response if someone's at risk from falls or infection.

The building has many features that make life comfortable, like private bathrooms, kitchenettes, air conditioning, fully furnished rooms, and access to cable TV, Wi-Fi, and in-room telephones. There's a dining room that can seat up to 50 people, where residents get meals prepared by a chef, offering options for allergies, diabetes, and all-day dining, which helps folks stick to their needed diet, and daily life includes laundry, housekeeping, and even dry cleaning to keep things tidy. The facility holds regular religious and spiritual support, especially Christian activities, for those who want them.

People living at Princeton Health & Rehab Center can join in scheduled daily events, community-sponsored activities, and even start their own group programs, and with spaces like a library, arts and crafts rooms, a beauty salon, garden, fitness and wellness rooms, games room, business center, outdoor spaces, walking paths, and a movie theater, there's a lot to keep residents busy. Transportation gets arranged for both medical appointments and local outings, and the facility strongly encourages that people stay active and social through fitness programs, wellness checks, planned day trips, and mental wellness supports. Residents and families also have access to counseling, and short-term respite care is available, giving family caregivers a break or providing a temporary stay for those who just need help between hospital and home.

As with many nursing facilities, this one does have a recent history of problems, including a fine of $386,750 due to deficiencies, with 9 health standard deficiencies and 6 health complaint deficiencies recorded, and it currently holds an average Medicare rating of 2.4 out of 5 stars based on evaluations-results that new management is working on improving. The staff keeps an eye on residents' health, watching for signs of trouble like infections, and responds promptly if someone has a sudden health issue. There's a focus on patient needs and a variety of activities, but it's not perfect and families should always ask specific questions about care and daily life during a tour, keeping in mind that improvements are underway. The facility accepts both Medicare and Medicaid, trying to make care possible for more people, and offers support for folks who want to maintain connections and their quality of life, aiming to create a warm and comfortable environment for those living there.
