    Beautiful building and compassionate staff

I love Elliott - the building is beautiful, spotless, and feels like home; the staff are warm, attentive, and treated my family member like family, providing excellent, compassionate care, therapy and engaging activities. I'd highly recommend it - a few reviews mention occasional communication or staffing issues, but our experience was very positive.

    Summary review

Overall sentiment in the reviews is strongly positive but punctuated by a number of serious, recurring concerns. The dominant theme across the majority of summaries is that Elliott Nursing and Rehabilitation offers caring, compassionate, and family-like care. Many reviewers explicitly praise nursing and caregiving staff as attentive, warm, and willing to "go above and beyond," describing residents as being treated with dignity, bathed and fed, and participating in many daily activities. The facility's cleanliness, fresh smell, and well-maintained building are repeatedly noted; multiple reviewers called the environment "spotless," "homey," or among the best in the region. Rehabilitation and therapy services receive favorable mention, with several accounts highlighting successful outcomes and helpful therapy that supported residents' recovery and return home.

Staffing and culture receive high marks from many respondents. Several reviews single out leadership for being accessible (open-door executive director), a knowledgeable director of nursing, a helpful social worker, and accommodating office staff. Community involvement, volunteer support, and a strong sense of teamwork and pride among staff are recurring positives. Activities and programming appear robust and engaging, with frequent references to many activities throughout the day, meaningful social interaction, and a welcoming memory care environment for residents with cognitive needs.

However, interspersed with these positive accounts are multiple reports of significant quality and safety concerns. A cluster of reviews alleges understaffing, caregivers on personal cellphones, and unresponsive staff resulting in neglected patients and multiple falls. There are several serious claims including falls that led to hospital transfers, delayed notification of families about hospitalizations, pressure ulcers/bedsores that families say were not disclosed, and at least one report of medication mislabeling. Some reviewers described canceled doctor appointments during stays and difficulty transferring residents to other facilities. These issues indicate inconsistent performance and raise safety-related red flags that contrast sharply with the many positive experiences.

Communication and management responses appear mixed. While some families praise an open-door executive director and responsive nursing leadership, others report administrators being unavailable and poor staff communication, particularly around critical events like hospital transfers or the development of pressure ulcers. Visitation restrictions and emotional responses from staff during losses were also mentioned in a few accounts; these may reflect specific incidents or times of heightened protocols rather than general practice, but they contribute to uneven impressions. Food service is another area of mixed feedback: many reviews do not complain, but several specifically note that meals were served cold and took a long time to be warmed, which is a recurring operational concern.

In summary, Elliott Nursing and Rehabilitation is repeatedly lauded for compassionate caregiving, strong rehabilitation programming, cleanliness, and a community-oriented atmosphere. Those seeking a facility may be encouraged by the many testimonials of attentive staff and successful rehab outcomes. At the same time, the reviews contain multiple and serious allegations around staffing levels, communication failures, safety incidents (falls, pressure ulcers), medication handling, and logistical problems (transfers and food temperature). These negative patterns, while not universal across reviews, are significant and warrant direct inquiry.

Recommendations for prospective residents and families: ask management about current staffing ratios and turnover, request details on fall-prevention protocols and recent fall statistics, inquire about skin and pressure-ulcer prevention and disclosure practices, clarify medication administration and labeling safeguards, confirm hospital transfer and family-notification procedures, and taste-test dining options or ask about meal warming practices. Families should also seek specifics about how administrators handle complaints and emergencies, and consider speaking with current residents or families for a range of recent experiences. The overall impression is that many residents thrive at Elliott due to devoted staff and good programming, but potential clients should verify safeguards and protocols to ensure consistent safety and communication aligned with those positive testimonials.

    About Elliott Nursing and Rehabilitation

Elliott Nursing and Rehabilitation sits at 20 Howard's Creek Rd in Sandy Hook, Kentucky, and the place has 75 beds for residents, most days you'll find about 71 people living there, and they offer skilled nursing care all day, every day, with folks there who help manage daily activities like eating, dressing, and moving around, plus they've got support for medication, IV therapy, diabetes management, and balance training. The building's got rooms with high-speed internet, and if someone needs help with things like x-rays, lab work, mental health talks, podiatry, private home care, or hospice, the staff can set that up. The care team includes specially trained people who work mostly with seniors and they make personal therapy and care plans to help folks keep their independence as much as they can.

You'll see a focus on rehabilitation, with short-term rehab, outpatient therapy, occupational and speech therapy, and programs for post-acute recovery, so if someone's coming from the hospital, that's something to know, and there's memory care, long-term care, hospice, and respite care, too. They handle cases where people need more medical attention, and their licensed nursing home services cover a range of support, though staffing has been a bit lighter, with 3.14 nurse hours for each resident compared to the state average of 3.9, but their nurse turnover rate is actually less than what you'll find in many places, sitting at 45.3%.

Some issues have come up-inspectors found they had not always met federal standards for infection prevention, and there were citations related to quality of life for residents, care planning, and specifically for handling and preventing pressure ulcers, including developing and following complete care plans for people who needed them. The owners are Elliott Nursing And Rehabilitation Holdings LLC, with David Marx being the indirect owner. The facility brings together rehabilitation services, skilled nursing, and different types of care for people who need ongoing support, and they work to cover a broad range of health and daily living needs for seniors in Sandy Hook. You'll find their rooms and shared areas set up for comfort, and there's a big focus on helping folks recover or maintain health, but always with the understanding that there have been areas where improvements were needed.
