    Excellent staff, systemic safety concerns

I had a mixed experience. The caregivers, therapists, and administrators I dealt with were compassionate, knowledgeable, and went above and beyond - the facility felt clean, welcoming, and great for rehab. However, I also saw or heard about serious safety and management issues (pests/mold reports, medication and handoff lapses, pressure-sore/incident concerns, poor communication and fear of retaliation), so while the staff is excellent, I'm cautious about fully recommending this place until those systemic problems are resolved.

    Summary review

Overall impression: Reviews of Maple Grove Senior Living, LLC are highly mixed, with a sharp division between families who praise the staff, environment, and rehab outcomes, and those who report serious safety, infection-control, and care-quality problems. A substantial number of reviewers describe the facility as warm, well-organized, and staffed by kind, professional caregivers who go above and beyond. At the same time, other reviewers allege alarming problems such as pest and mold infestations, clinical neglect, communication failures, and management denial. The result is a polarized reputation: many families feel confident recommending the facility, while others strongly advise against it due to safety concerns.

Staff and caregiving: One of the clearest patterns is frequent, repeated praise for frontline staff. Numerous reviews describe caregivers, nurses, and therapists as compassionate, patient, and knowledgeable. Specific personnel are commended — therapist "Katie" is singled out for excellent rehab work, and multiple accounts praise an on-site administrator for hands-on leadership, 24/7 accessibility, and compassionate involvement. Several family members recount staff going above and beyond (including weekend support), strong organization, and memorable, loving care at end-of-life. That said, there are also serious personnel-related complaints: confrontational behavior is alleged in at least one instance (name mentioned), staffing/workplace concerns by employees were referenced, and some reviewers noted inconsistent personal care and unequal access to physical therapy.

Clinical care and safety: A concerning cluster of reviews report significant clinical lapses. Examples include an untreated diabetic patient for six days, severe pressure sores, alleged improper handling leading to acute events (a bed incident with subsequent brain bleed and heart attack is reported by reviewers), and instances of hospital transfer or death without adequate family notification. Several reviewers explicitly describe neglect or mistreatment, and some strongly recommend against the facility for safety reasons. Conversely, other families report successful rehabilitation and recovery with good nursing care; one review states the facility improved after staff education. These conflicting accounts suggest variability in clinical performance — some residents receive excellent care while others experience critical failures.

Infection control, pests, and environment: Reviews diverge sharply on the physical environment. Many describe Maple Grove as a beautiful, cozy, and non-intimidating home with large formal living areas, lovely furnishings, fireplaces, and big TVs — attributes that contribute to a welcoming atmosphere. Multiple reviews call the facility very clean and orderly. However, another group of reviews alleges serious environmental problems: bed bugs, roaches, mice (including a mouse in a resident room), bats, and black mold are all reported. Outbreaks of scabies and shingles and related quarantines are also mentioned. These allegations raise red flags about infection control and maintenance. The juxtaposition of descriptions — attractive, home-like spaces versus reports of infestations and mold — points to inconsistency, possible recent remediation by management, or variation between units/areas.

Management, communication, and operations: Management gets both strong praise and strong criticism. Positive reviews emphasize a hands-on administrator who is compassionate, knowledgeable about operations, and reachable; some families say the administrator became like a friend and that the director went above and beyond. Negative reviews, however, accuse management of denying problems, intimidating or retaliating against complainers, and failing to notify families about important events (quarantine measures, hospital transfers). Complaints also touch on billing and financial disputes and an alleged HIPAA violation; there are even claims of fake or deceptive reviews. Operational concerns such as construction-related closures and discrepancies in therapy access are also mentioned. Taken together, these comments indicate that while leadership can be effective and visible, there are also perceived leadership or cultural issues that undermine trust for some families.

Patterns, implications, and guidance: The most significant pattern is variability. Multiple reviewers endorse the facility strongly — citing compassionate staff, effective rehab, cleanliness, and a warm environment — while a smaller but serious subset reports dangerous lapses (infestations, mold, untreated medical conditions, inadequate notifications, and alleged retaliation). Because some issues reported are of high risk (infection control, untreated diabetes, pressure ulcers, and serious safety incidents), prospective residents and families should treat these allegations seriously. Key steps for due diligence would include asking management for recent inspection or infection-control records, evidence of pest remediation and mold abatement, clarification of hospital-to-facility transfer protocols, staffing ratios and therapy schedules, policies for family notifications, and examples of how past complaints were investigated and resolved. It is also reasonable to request references from current residents' families and to verify staffing credentials and turnover.

Bottom line: Maple Grove receives strong, heartfelt praise for many aspects of care and the physical setting, anchored by repeatedly praised staff and a visible administrator. However, the presence of multiple, specific allegations about pests, mold, infection outbreaks, clinical neglect, communication failures, and potential management cover-ups represents significant cause for concern. The reviews point to a facility that can provide excellent, loving care for some residents but has had reported, potentially serious failures for others. Families should weigh both the positive testimonials and the safety- and clinical-related complaints carefully, verify current conditions and corrective actions, and monitor individual care plans closely if choosing Maple Grove.

About Maple Grove Senior Living, LLC

Maple Grove Senior Living, LLC sits in the countryside around Shelbyville, Kentucky, with a stately front porch, gazebos, courtyards, and beautifully landscaped outdoor areas giving it a peaceful feel, while the building's recent renovations help make things more comfortable and bring the residents together. This place is a non-profit skilled nursing home, licensed for 117 beds, with about 80 people living there every day. The staff, ranging from 51 to 200 employees, includes experienced professionals, therapists, and skilled nursing care professionals who work to meet both medical and emotional needs, which is something you do notice-since many stay long-term, there's a sense of familiarity here. Nurse aides provide about 2 hours and 37 minutes of care per resident daily, with licensed nurses averaging 4 hours and 33 minutes per resident each day, which is well above the Kentucky average, and nurse turnover's also a bit lower than in most places. You see about 3 minutes of physical therapy per person each day, so therapy's available though not for long periods.

Maple Grove has won several awards for long-term care and's recognized as the most awarded senior living and long-term care facility in Kentucky, plus it sticks close to the old Masonic tradition in its values. It's not joined to a hospital or a big retirement community, so it stays a bit quieter and more focused, which matches well with what people look for in the area. The building passed its last fire inspection in July 2024, has had no Medicare payment denials or federal fines in three years, and hasn't gotten complaints that led to citations. The most recent inspection showed four deficiencies-which is about average-including a couple for infection control and one about food sourcing, but there was no actual harm, just the potential for things to get worse if not fixed. It has a health inspections quality rating of 4, with an overall rating of 4 out of 5, but for resident care quality the score's around 2 out of 5 for short-stay and 3 for long-stay residents, which is something to think about.

The community offers several kinds of care: skilled nursing, long-term stays, assisted and personal care, respite services if someone stops in for a short stay, rehab services, and memory care programs for people who need help with memory loss. Residents and families can join councils to share their thoughts and help with decisions. There are activities to keep people engaged, and the place encourages connection and trust, trying to make it homelike as much as possible. Maple Grove is nonprofit and privately held and puts an emphasis on compassionate care and community involvement. For those who'd like to look around before coming, it features a virtual tour and photos of the community. Overall, Maple Grove Senior Living, LLC focuses on steady care, safety, and a sense of belonging, with a mix of practical support and attention to comfort, though it's smart to check what features and services are currently offered since there's not a lot of detailed information about them out there.
