    Caring staff but unsafe cleanliness

I saw genuine care - friendly, helpful staff, excellent therapy/rehab, engaging activities, and happy residents. But I also experienced neglectful nursing: dirty clothes/diapers, foul odors and pests, unresponsive call lights and slow medical response. Leadership and several staff (Brittany, Ms. Robinette/Team Claiborne) went above and beyond, yet staffing shortages and inconsistent oversight left dangerous gaps. Overall: kind people and great rehab, but serious cleanliness and reliability concerns - visit carefully before deciding.

    Summary review

Overall sentiment across the reviews for Claiborne Healthcare Center is highly mixed, with strong, repeated praise for staff, therapy services, activities, and certain administrative leaders, contrasted by alarming reports of neglect, cleanliness and infection-control problems, and inconsistent clinical responsiveness. Many reviewers describe a warm, family-like atmosphere where specific staff members and leaders (Brittany, Keisha, Peaches, Ms. Robinette and others) are singled out for exceptional compassion, professionalism and hands-on involvement. The therapy/rehabilitation department receives frequent commendation for going above and beyond to help patients recover and regain independence. Activities, entertainment, volunteer opportunities, and outdoor/patio spaces are commonly mentioned as positive features that contribute to residents’ enjoyment, improved mood, and a sense of community. Multiple reviewers describe residents who are happy, smiling, and socially engaged, and some families report excellent clinical care, responsive administration, and a facility that feels safe and welcoming.

However, a significant and troubling portion of reviews report the opposite experience: allegations of dirty conditions (including strong odors, filthy areas, and complaints of roaches, flies, and moldy food), laundry and closet mismanagement (clothes left lying around, dirty hampers), and soiled linens/diapers. Several reviews describe neglectful practices such as failure to check vitals, refusal to provide water, residents not eating for extended periods, unresponsive call lights, and nurses who are inattentive or dismissive. There is at least one serious report claiming an unlisted medication was administered to sedate a resident and other accounts that family concerns were ignored. These reports raise concerns about infection control, resident dignity, and basic daily-care practices.

Clinical quality and medical responsiveness are also described inconsistently. Many reviewers commend the nursing staff and clinical teams for professional care and timely therapy, yet others describe slow medical response, delayed physician visits, doctors who dismiss symptoms, and instances where worsening conditions required emergency room transfers. These conflicting accounts suggest variability in clinical performance across shifts, units, or patient populations. Staffing issues are frequently mentioned as a likely contributor: reviews point to shortages, turnover, and insufficient staff-to-resident ratios, and some families perceive management as indifferent or profit-driven when complaints are raised. A handful of reviewers said complaints to owners or state were ignored, which compounds family frustration and erodes trust.

Dining receives mixed feedback: some reviewers praise family dining invitations and the social aspects of meals, while numerous comments criticize food quality and call for a new menu. The facility’s physical environment is similarly polarized: many reviewers note new furniture, an attractive, clean atmosphere with pleasant smells and well-kept outdoor seating, whereas others report filthy conditions, odors, pests, and moldy food—issues that, if accurate, are serious and require immediate remediation.

Patterns that stand out are the strong, repeated praise for individual staff members and the therapy department alongside recurring, serious allegations of neglect, cleanliness failures, and inconsistent medical care. This bifurcation suggests that care experiences at Claiborne Healthcare Center may depend heavily on specific staff, shifts, or units. For families considering this facility, the reviews indicate potential for excellent rehabilitation outcomes, compassionate caregivers, and a community atmosphere, but also carry red flags requiring diligence: verify staffing levels, infection-control practices, medication administration policies, laundry and dining operations, and responsiveness to family concerns. Prospective residents and families should ask to see recent state inspection reports, inquire about lists of recent complaints and corrective actions, and, if possible, spend time observing multiple shifts to assess consistency.

In summary, Claiborne Healthcare Center is portrayed as capable of providing outstanding, compassionate, and effective rehab and residential care in many instances, driven by dedicated staff and engaged leadership. At the same time, multiple reviews describe potentially dangerous lapses—neglect, sanitation and pest problems, medical response delays, and inconsistent care—that warrant caution. The facility may offer an excellent environment where staffing and management perform well, but the variability and severity of negative reports justify careful, specific questions and ongoing monitoring by families and oversight agencies.

The center offers long-term care and short-term respite care for older adults and people who need rehab, with 81 certified beds and usually around 64 residents each day. Nurse staffing runs at about 3.36 hours per resident per day, which falls a little below the Louisiana average, and the state has found 44 deficiencies here during inspections, some of them tied to issues with abuse, neglect, and policy lapses, so it's good to know they've had to address these problems in the past. Since January 2002, Nexion Health, Inc. has managed Claiborne Healthcare Center, with the Nexion Neighbor Program and staff trained in both skilled nursing and long-term care.

The facility's team includes an interdisciplinary care group with a mix of nurses and specialists offering wound care, daily rehabilitation like physical, occupational, and speech therapy, memory care for dementia and Alzheimer's, and telemedicine services for easier access to healthcare. There's always nursing care around the clock, and admissions stay open 24 hours a day, seven days a week, which helps families during emergencies or changes in care needs. They work to tailor care for each resident, providing dietary and nutrition guidance, custodial help, hospice support, and programs meant to protect the comfort and dignity of everyone who lives there.

Life at Claiborne Healthcare Center includes simple comforts and activities like a movie theatre, chapel, beauty and barber shop, along with activity programs run by an Activity Director, and residents can bring their pets, which helps keep things feeling home-like. You'll see a strong local presence and a reputation for compassion and community, with their team focusing on kindness, emotional support, and safety, which is important for families and residents to know. The center sits near several big hospitals, making it easier for residents to get specialized healthcare if they need it. They pay attention to infection control and respond to complaints during inspections, which is especially important in nursing and rehab settings.

Claiborne Healthcare Center reports a review score of 4.5 based on 23 reviews online, and they've received the Step 1 Quality Award from the American Health Care Association and the National Association for Assisted Living.
