    Summary review

Overall sentiment across the reviews is strongly positive, with repeated emphasis on cleanliness, respectful and compassionate staff, and a safe, pleasant environment. Reviewers consistently note that rooms and the facility are clean, fresh-smelling, and well maintained. Housekeeping and maintenance receive direct praise; specific mentions include made beds, fresh-smelling rooms, and timely light repairs. The facility’s atmosphere is described as beautiful, controlled, and organized, which reassures families and visitors and contributes to a favorable first impression.

Care quality and staff performance are major themes and are portrayed very favorably. Multiple reviewers highlight attentive nurses and CNAs who demonstrate genuine care — examples include improved resident appetite and clearer skin, residents being in good spirits, and family members feeling grateful for the caregiving. Staff are characterized as warm, caring, professional, and knowledgeable. Named staff such as Monique, Paige, and Rose were singled out for helpfulness and hands-on care, and many reviewers commented that staff explained details well and maintained open communication. Families appreciate proactive treatment planning, regular care plan conference calls, and that staff keep relatives informed about daily care and changes in condition.

Communication, admissions, and administration are described as smooth and supportive. Several accounts point to a helpful and friendly admission process and positive interactions with administrative personnel. Reviewers repeatedly mention clear, informative staff communication — staff are said to answer questions, explain details “fantastically,” and involve families in planning. This organizational approach appears to foster trust and optimism; many relatives expressed confidence that their loved ones were settling in and receiving appropriate attention.

Resident experience and programming also receive favorable remarks. The facility attempts to involve residents in activities and runs monthly engagement opportunities; reviewers note that residents are happy, engaged, and treated with respect and dignity. Staff make efforts to encourage dining participation (including negotiating to bring residents to the dining room), contributing to improved appetite and overall wellness. Spiritual care and ministry are another distinct strength cited by several reviewers: staff or volunteers provide uplifting ministry, inspiration from faith, and a comforting spiritual presence for those who value it.

Dining and routine care elements are generally positive: visitors observed that residents had eaten, were alert, and in good spirits after meals. Reviewers mention a focus on healthy routines, enjoyable meals, and responsive care. The combination of proactive treatment plans and hands-on assistance appears to yield tangible improvements for some residents.

Negative comments are sparse and generally minor. A few reviews described occasional “hiccups” in operations, but emphasized these issues were resolved promptly. One specific maintenance item (a burnt-out light in an identified room) was noted, yet maintenance responsiveness was otherwise praised. Some families reported initial hesitation or an adjustment period before feeling fully comfortable, which is common when transitioning to a new setting. The facility has a smoking area available on-site — welcomed by some but potentially a concern for others. There are no recurring reports of major clinical or safety problems in the provided summaries.

In summary, the aggregated reviews portray Audubon Health & Rehab as a clean, well-run facility with a caring and communicative staff, effective housekeeping and maintenance, and programming that supports resident engagement and spiritual needs. Strengths include strong interpersonal care from nurses and CNAs, transparent communication with families, a pleasant environment, and evidence of residents’ improved well-being. Areas to monitor are minor operational glitches and the management of on-site smoking for those sensitive to it, but reviewers consistently note that staff address issues promptly. Overall, families repeatedly express gratitude, satisfaction, and willingness to recommend the facility.

    About Audubon Health & Rehab

Audubon Health & Rehab has served the community for over 40 years and provides nursing home care, rehab services, and help for seniors who need extra support with daily life, and being known also as Audubon Guest House in Thibodaux, LA, it's a place where people can get both short-term rehab after things like surgery or illness and longer-term care for those who need it, and memory care is available for people with Alzheimer's or other memory problems, with a specially designed area to reduce confusion and help prevent wandering. The facility offers many services, including skilled nursing, physical rehab, occupational therapy, speech therapy, respiratory therapy, and wound care, and it also provides hospice and palliative care.

The community focuses on a resident-centered approach, treating people with dignity and respect, and staff try to give attention that fits each resident's own needs while encouraging activities that help with social, physical, and emotional well-being. Though Audubon Health & Rehab has received several awards, including being recognized as one of America's Best Nursing Homes by Newsweek and honors for meals, activities, and friendliness, inspection reports show it's had some problems. The facility's had nine deficiencies found over three inspection reports, including citations for infection control, failing to keep areas free of hazards, and for not having a full infection prevention program, but staff use enhanced barrier precautions to help keep residents safe. Nurse turnover is 26.1%, lower than the state average, though nurse staffing is just below the state average.

Management has changed, with Pathway South LLC and Providence Care, LLC at the helm in the past, and it's owned by Medico LLC and Act Investments, LLC, with ties to Beebe Family ownership. With 172 certified beds and around 116 residents per day, the facility tries to offer a supportive space for recovery, rehab, and ongoing care, aiming to deliver help and specialized care to seniors in a simple and straightforward way.
