    Excellent care but poor communication

I have mixed feelings. The staff and therapy are excellent - my aunt is well cared for, active with engaging programs, the grounds and meals are lovely, and the facility shows real kindness and pride. But I was heartbroken when personal belongings were removed without notice (some missing) and staff seemed unaware; I also noticed occasional urine odor, slow assistance that suggests understaffing, and unclear pricing/exclusive vibe. Overall great care, but poor communication and transparency left our family distressed.

    Summary review

Overall impression: The reviews for Autumn Leaves Nursing & Rehabilitation Center, LLC show a predominantly positive sentiment centered on compassionate care, strong rehabilitation services, an active programming schedule, and a clean, attractive environment. Many reviewers emphasize that staff are kind, respectful, and invested in residents’ well-being. Rehabilitation and therapy are repeatedly praised as among the facility’s best features, and family members often express gratitude and pride in the care their loved ones receive.

Care quality and staff: A consistent theme is that the clinical and caregiving staff are caring and attentive. Multiple reviewers specifically call out nursing and office staff for their compassion and respect, and several note that residents seem happy and well cared for. The therapy/rehab department receives strong, repeated commendations for the frequency and quality of therapy sessions. At the same time, there are isolated but notable reports of rudeness and poor treatment — not of residents but of employees — and at least one account of a loyal employee being neglected. These negative reports suggest some internal staff-relations or management issues that coexist with the generally positive caregiving culture.

Facilities, cleanliness, and activities: The facility’s physical environment is frequently praised: clean, well-maintained spaces, pleasant smells, private rooms, and attractive/gorgeous surroundings. Activities are described as engaging and varied — highlighted examples include musical programming such as a grand piano and singing — and families report that residents enjoy and participate in these activities. Meals are noted positively in multiple summaries. These features contribute to a family-focused, home-like atmosphere for many residents and their relatives.

Communication, management, and logistics concerns: Despite the many strengths, several reviews raise serious operational and communication concerns. The most acute issue reported is the removal and disposal of a resident’s personal belongings without notifying family members or giving them a chance to retrieve items; staff were reportedly unaware this occurred, and the incident caused emotional distress and heartbreak. Other communication-related concerns include unclear or inconsistent responses to family requests (including ignored donation requests) and an overall sense of poor communication from management in some instances.

Hygiene and staffing concerns: While the facility is often described as very clean, there are multiple mentions of urine odor in patient rooms and difficulty obtaining assistance, which reviewers associate with potential understaffing. These comments suggest that while public and communal areas may be well maintained, some resident rooms occasionally experience lapses in odor control or timely assistance, and that staffing levels or responsiveness may fluctuate.

Transparency and image: A recurring theme for some reviewers is the perception of exclusivity: rates are not posted, producing pricing-transparency concerns, and the facility projects an image that some interpret as wealth-focused. These impressions can create barriers or unease for prospective residents and families seeking straightforward information about costs and accessibility.

Overall balance and implications: In sum, most reviewers report excellent, compassionate care, effective rehabilitation services, active programming, good dining, and a pleasant physical environment. However, the reviews also surface several serious and recurring concerns — most notably the mishandling of personal belongings, intermittent odor issues, potential understaffing and delayed assistance, lapses in communication, perceived employee mistreatment, and lack of pricing transparency. These negative items are fewer than the positive comments but are significant because they affect resident trust, family peace of mind, and perceptions of management competence. Addressing the communication failures, ensuring consistent housekeeping and timely resident assistance, clarifying pricing, and improving internal staff relations would likely reduce the most serious criticisms while preserving the facility’s many strengths.

About Autumn Leaves Nursing & Rehabilitation Center, LLC

Autumn Leaves Nursing & Rehabilitation Center, LLC offers care for older adults who need help with daily living, skilled nursing, or support after illness or surgery, and there you'll find special programs for Alzheimer's and Dementia because they know different folks need different kinds of help and that's something they've paid careful attention to over the years, so if someone needs respite care, hospice care, or social services, the staff know what to do and always try to be gentle. The building has a modern look and it's one of the most spacious places around this part of the South, giving folks room to move about and gather for daily activities, which residents often say are a real highlight because there's plenty to do and even church services for those who want them, and the therapy department draws good words from people who've managed to go home after doing physical, occupational, or speech therapy, sometimes after working with several types of therapists at once. Supervisors take their jobs seriously, making sure staff pay attention to more than just the routine, remembering how meaningful a simple kindness or a quiet talk can be when someone's dealing with changes, and this tradition goes back to the resident care principles started by Ted and Marjorie Price who believed in gentle, heartfelt compassion. The rooms include comforts that help people feel at home during recovery or long-term stays, and everyone works together-staff, supervisors, and all the helpers-so that support is there both for residents' health and for their emotional well-being.
