About Somerset Rehab & Living Center

Somerset Rehab & Living Center sits in the North Country community and offers many types of care, like assisted living, skilled nursing, independent living, and memory care for folks who have confusion or wandering risk, and you'll also find long-term nursing care, short-term skilled rehab, post-acute and transitional care, and even respite and hospice care, so it's a place that covers plenty of needs. The building is a single story, making it easier for folks to get around, with bedrooms and common areas all on one level, plus function rooms for meetings, a dining room, furnished rooms, walking paths, a garden, and an enclosed area for picnics and gatherings. The team includes licensed practical nurses, registered nurses, certified nursing assistants, and staff offer 24-hour supervision, so there's help with bathing, dressing, transfers, medication management, and even on-call support nearby, and they'll coordinate with outside healthcare providers too. The community has just 21 beds, so it's not too big, and they take Medicare and Medicaid payments if you qualify, which is helpful for many. If you need therapies, Somerset has it covered with physical therapy, occupational therapy, speech/language pathology, sports medicine, and vestibular evaluations for dizziness or vertigo, all available both inpatient and outpatient, and doctors help direct each person's care plan. Residents also get dietary services with meals planned by a registered dietitian and the food service supervisor, with input from residents-there are dining rooms, scheduled meals, all-day dining, snacks, and meal options for those with allergies or diabetes. Housekeeping, laundry (including dry cleaning), pharmacy services, clinical labs, X-rays, podiatric care, and medication support keep everyone comfortable and safe, and move-in coordination is available to make transitions easier. Social life comes with activities and lots of community-sponsored events, like movie nights, group discussions, outings for lunch or shopping, regular films, and sometimes Meals on Wheels or blood pressure screenings, so you can stay active if you like, and exercise classes are available too. Optional amenities you can pick add private phone lines, cable TV, and even in-house hairstyling, and folks who want more privacy or support can choose different living arrangements based on how much care or independence they need. Parking and transportation help are available, and you'll also find an emergency alert system throughout the building for safety. The North Country feel is present in the atmosphere and in how the community comes together, and programs focus on each person's needs, working with their goals under the guidance of a physician, and everyone gets maintenance-free living with less worry about chores, meal prep, or home upkeep.
