    Caring staff, clean, minor issues

I placed my mom here after a miserable prior stay and overall I'm grateful: the staff are genuinely caring, professional and helpful with transfers/bed and wheelchair needs, nursing and PT were excellent, meals and activities (bingo, music, sun room and outdoor areas) kept her engaged, and the place felt clean and gave us peace of mind. Admission and discharge planning were smooth and the room was functional, though the building is older and could use cosmetic updates; occasionally phone/communication and cleaning lapses disappointed me. Despite that, I highly recommend this facility for the attentive, compassionate care and strong staff-resident relationships.

    Summary review

Overall sentiment across the review summaries is mixed-to-positive, with a strong majority of reviewers emphasizing caring, professional, and supportive staff and a subset expressing serious concerns about cleanliness, responsiveness, and occasional neglect. The most consistent positive themes are the interpersonal aspects of care: reviewers repeatedly cite compassionate nursing and caregiving, close relationships between staff and residents, a home-like atmosphere, and peace of mind for families. Multiple families highlight that staff go "above and beyond," provide effective physical therapy that helped residents return home stronger, and offer supportive end-of-life care. These reports suggest that clinical and rehabilitative services are meaningful strengths for many residents.

Facility features and programming receive largely favorable comments. The presence of a sun room, outdoor areas, and a variety of activities including bingo and music are frequently mentioned. Reviewers note that residents are not confined indoors and appreciate opportunities for social interaction. Several reviews call out an improved dining room setup and the availability of alternatives at mealtimes; afternoon coffee and generally enjoyable meals are singled out positively by many. The staffing level is often described as favorable (good staff-to-resident ratio), contributing to perceptions of individualized attention and cleanliness in many parts of the facility.

However, there are notable and recurring negative themes that temper the positive impressions. Cleanliness and maintenance are inconsistent in reviewers' accounts: while some describe the facility as clean and well-kept, others report unclean bathrooms, rooms not being cleaned, and an overall depressing or cold structure. The building's age and need for cosmetic updates are explicitly mentioned, and at least one reviewer suggested practical safety improvements such as installing pull-up bed rails. Food quality is another area of variability — several reviewers praise the meals, yet others call the food "terrible," indicating inconsistency in dining satisfaction.

Communication and reliability concerns appear across several summaries. Some family members report unresponsive staff, phone calls not being answered, and poor communication from management or nursing. More severe allegations are present in a minority of reviews: reports of neglect, use of restraints, lack of basic grooming (e.g., not shaved), and a perception that some staff are careless or motivated by money rather than resident welfare. These serious criticisms contrast sharply with the many accounts of excellent nursing and compassionate care, suggesting possible variability by shift, unit, or time period.

In sum, Pleasant View Healthcare Center appears to provide high-quality, compassionate care for many residents, with strengths in staff–resident relationships, therapy services, a range of activities, and a generally reassuring environment for families. At the same time, there are consistent reports of unevenness — particularly regarding cleanliness, food, building condition, and communication — and a small but serious set of allegations about neglect and poor treatment. Prospective residents and families would be well served by touring the facility, observing cleanliness and mealtime, asking about staffing patterns and restraint policies, inquiring how management addresses complaints, and clarifying whether recent improvements (e.g., dining room access) have been sustained. This mixed but predominantly positive pattern suggests a facility that excels in human-centered care but may have operational or consistency issues to monitor closely.

    About Pleasant View Healthcare Center

Pleasant View Healthcare Center sits over in Mount Airy, Maryland, and offers many kinds of care for seniors and others who need help. They've got skilled nursing care, assisted living, memory care for dementia and Alzheimer's, and they do short-term rehab, long-term stays, palliative and hospice care, and respite options when someone's regular caregiver needs a break. The place is part of CommuniCare, a family-owned company, and ties in with MedStar Health too. There are 104 certified beds, but about 72 residents on average live there each day, so the facility isn't always at full capacity, and you'll find the doors open 24 hours a day, every day of the week, with ADA accessibility for folks who need it, so anybody, any age, can come in.

The nurse turnover rate is 21.2%, much lower than Maryland's average, but staffing runs about 3.39 nursing hours per resident each day, which is lower than the state's typical 3.9. They offer therapy services-physical, occupational, and speech-as well as psychotherapy, group and individual counseling, and long-term rehab programs. You'll find a cafeteria for meals, along with planned menus, nutrition counseling, and meal prep, plus there's a beauty salon and barbershop, which can be nice if you want some normalcy. Residents can enjoy simple things like bingo, board games, special holiday parties, group walks and outings to movies or shopping. Everybody gets help with things like bathing, walking, dressing, and the staff even help write letters if needed. They clean rooms and do laundry, so families won't have to worry about those chores. You'll come across various programs the facility offers, like Honor 360 and Advance 360, which focus on personalized care and attention.

The place ran into trouble in recent inspections, though, because they received citations for not ensuring privacy and good access for residents to communicate with people outside and for not keeping certain areas safe enough to prevent accidents. The inspection in May 2023 found 35 deficiencies, including one tied to infection control, and the state fined Pleasant View Healthcare Center $15,593. Inspections have run late, too; the latest standard check was over two years ago instead of the usual 9 to 15 months.

Ownership runs through OMG MSTR LSCO, LLC, managed by Health Care Facility Management, LLC since January 2022, and some indirect owners are C R Stoltz II LLC and C.R. Stoltz Family Investment Company Inc. The staff includes Bill Horgan as Admissions and Marketing Director, Cyndi Free as Human Resources Director, and Shellie Hoffman as Executive Director. The care team tries to involve both residents and families in choosing care, and you'll see a supportive environment where folks can join in daily activities or simply get the help and supervision they need. The overall setup aims for comfort, personal attention, and letting residents keep as much independence as possible, even if it's not perfect and problems have turned up in state reports.
