    Unclean understaffed facility; neglectful care

    I stayed here for rehab and my experience was mostly terrible. The place was unclean (soiled rooms, gross bathrooms, no fresh linens), poorly managed and understaffed, which led to neglectful care - missed baths, soiled stays, overmedication, safety incidents, delayed responses and even a hospitalization. Communication and records requests were ignored, food was poor (diabetic brownies not sugar-free), and items went missing. A few CNAs, therapy staff and the DON worked hard and were caring, but overall I would not trust this facility with a loved one - do not send them.
