    Kind staff dishonest billing issues

I have mixed feelings. The staff were mostly kind, helpful and attentive - nurses respectful and great in my loved one's last days, DNR honored - and activities, snack cart and a pleasant dining atmosphere gave real peace of mind. However social services were dishonest/abusive and we've been fighting massive billing charges; nursing felt barely adequate at times because of long hours/high resident-to-nurse ratios. The facility and food were inconsistent (some newly remodeled, very clean areas with good meals; other parts run-down with poor food); the Alzheimer's unit is locked and rooms may be shared. I can't fully recommend it despite many caring staff.

    Summary review

Overall sentiment across the reviews is mixed but leans toward appreciation for frontline staff and the atmosphere in many parts of Libby Care Center, with significant concerns about consistency, management, and certain operational areas. Multiple reviewers repeatedly praised the caregiving team — describing staff as helpful, friendly, caring, and attentive — and emphasized respectful nursing communication and thoughtful end-of-life care where DNR wishes were honored. Many reviewers specifically noted a pleasant dining atmosphere, ample activities (bingo, crafts, church services), and elements that provide families with peace of mind, such as an Alzheimer's special care unit and what some described as newly remodeled, clean, and attractive areas of the facility.

Staff and direct care: The most consistent positive thread is the quality of interpersonal care from aides, nursing staff, and other frontline employees. Reviewers used words like "amazing," "helpful," and "attentive," and several accounts point to nurses and staff who explain care clearly and respond respectfully to resident needs. There are also testimonials of comfort in residents' final days and staff honoring DNR orders — important indicators of compassionate care and clear communication in critical moments.

Facility and environment: Opinions about the physical plant and upkeep are polarized. Several reviews describe the facility as beautiful, newly remodeled, and very clean, which suggests investments or upgrades in at least parts of the campus. Conversely, other reviews describe the facility as worn out and run down. This disparity suggests variability by unit or differences in reviewers' expectations and timing; prospective families should note the inconsistency and consider touring specific units. The Alzheimer's unit is mentioned as a locked/monitored area — a positive for safety-minded families but also described by some as restrictive.

Dining and activities: Dining impressions vary widely. Some reviewers praise the food as great or top-notch and appreciate amenities like a snack cart and a pleasant dining atmosphere, often crediting staff interaction for the positivity. Other reviewers found meals unacceptable — one extremely negative comment stated the meals were "not fit to feed pigs." Activity programming receives positive mentions overall, with multiple references to frequent and varied activities that help residents stay engaged.

Clinical staffing and operational concerns: Several serious operational concerns appear repeatedly. A key negative theme is understaffing of nursing personnel, described as a high resident-to-nurse ratio and long working hours, with at least one review calling the nursing "barely adequate." This raises questions about workload, staffing levels, and the potential impact on care consistency. Another significant concern centers on social services and management: one review accuses social services of abusive and dishonest behavior, and another reports a major billing dispute involving massive charges. These management-level issues — particularly billing conflicts and allegations regarding social services — stand apart from the generally positive impressions of direct care staff and may indicate administrative or systemic problems that families should investigate.

Patterns and recommendations based on reviews: The dominant pattern is that direct care staff are a strong point for the facility, while administrative consistency (billing, social services) and certain operational areas (food quality, facility condition in some parts, staffing levels) are inconsistent and can be problematic. Reviewers' experiences vary significantly, suggesting that outcomes may depend on the particular unit, time period, or staff on duty. In summary, Libby Care Center appears to offer strong, compassionate frontline care and a variety of activities for many residents, but there are documented and serious concerns around management practices, staffing adequacy, and inconsistent facility/dining quality that prospective residents and families should clarify before committing.

    About Libby Care Center

Libby Care Center, also called Libby Care Center of Cascadia, sits at 308 E 3rd St in Libby, Montana, offering a wide range of nursing home and rehab services in a quiet, rural setting, where the staff and nurses work hard to make residents feel safe and comfortable. The center runs under Cascadia Healthcare as a for-profit company and holds 101 certified beds, though about 79 residents stay each day, and as of June 2025, 21 beds are open. Managerial control's been with Warren Bodily since January 2017. There are nurses around 24/7, with nurse staffing at roughly 2.91 hours per resident per day, and a recently reported nurse turnover rate of 32.8%. Certified nursing assistants and other professionals help with bathing, dressing, and transfers, and medication management's handled on site, plus residents can always get help through a 24-hour call and emergency alert system.

The facility's well-suited for both short-term rehabilitation and long-term care, offering physical, occupational, and speech therapy every day, and it's got a rehab gym for people working to regain strength or movement, with therapists and physicians coordinating care plans for every person. You'll find private and semi-private rooms, all fully furnished with private baths, air conditioning, cable TV, kitchenettes, phone hookups, Wi-Fi, and even options like studios. There's a range of common areas like lounges, a game room, a cozy library, a fitness room, as well as outdoor patios, walking paths, and landscaped gardens, so residents can relax either indoors or outside. Residents get meal services with restaurant-style dining run by a professional chef, including choices for special diets like diabetic and allergy-friendly foods.

The place has community-sponsored activities, movie nights, music programs, and resident-run events, with transportation, daily scheduled events, family support, laundry, move-in help, and housecleaning included. There are welcome bags for newcomers, and staff are known for being attentive and providing good food. Both resident and family councils meet to discuss and improve quality of life at the center, and it's part of a larger Continuing Care Retirement Community (CCRC), which means seniors have access to several levels of care as their needs change. The rural location brings a peaceful setting, and recent renovations have kept the facility clean and updated.

Libby Care Center accepts Medicaid and Medicare, with both short- and long-term care covered. It's won the 2023 Silver - Achievement in Quality award from the American Health Care Association and National Center for Assisted Living. While the staff works hard, the county inspections have found 15 deficiencies, including one for infection control, safe environment, meeting professional standards in assessment and care planning, and behavioral health care services. The team focuses on patient-comfort, health, and dignity, and they're always looking for ways to keep everyone safe and engaged, with a code of conduct, privacy policies, and activities to suit different interests. For some additional services, occupational therapy from All About Kids Occupational Therapy's also available, mainly helping children in several areas, though the main focus stays on senior care. The facility's known for its clean, comfortable environment, community spirit, and variety of amenities, though like any care center, there's always room to adapt and improve to meet the needs of those who live there.
