    Compassionate memory care, supportive staff

My grandpa received memory care here and I'm deeply grateful - the nurses and staff were compassionate, affectionate, and attentive, worked well with hospice, and even helped my family through his funeral. Their sunny dispositions, genuine care (and visits from Hank the dog) made visits joyful; I'd gladly recommend this warm, supportive community, with just a small room for improvement.

    Summary of reviews

The Meadows at Ashland elicits a strongly mixed picture. Across the reviews there is consistent praise for the direct-care staff: many families describe caregivers as compassionate, attentive, and respectful, and they highlight strong resident–staff relationships. Memory-care programming and collaboration with hospice are noted positively, and specific elements such as pet visits and staff involvement in activities contribute to resident well‑being and family satisfaction. Multiple accounts credit the caregiving team with improving residents’ quality of life and offering supportive end-of-life assistance.

At the same time, reviewers point to several operational and environmental weaknesses that prospective families should consider. Cleanliness and maintenance issues in interior and common areas are recurring themes, including concerns about sanitation and odor control. These accounts are often paired with descriptions of inconsistent hygiene and incontinence-care support and at least one mention of interrupted utility/service availability that affected basic care tasks. Staffing levels are described as tight in ways that can delay routine care; reviewers link understaffing to slower response times and to burdens on aides.

Safety and administrative processes show notable variability. There are claims suggesting gaps in transportation and transfer-safety practices, and reviewers raised concerns about infection-control coordination during roommate changes, which they felt increased exposure risk in one instance. Financial and administrative transparency is another weak point: reviewers cite unclear billing practices and there is an allegation of financial misconduct by staff/administration. Family communication and admissions/check-in procedures are described as cumbersome or inconsistent, and one review specifically criticized the communication tone of leadership — a pattern that could affect families’ ability to get timely information.

Information about dining was limited in the available summaries; reviewers did not consistently comment on food quality or meal service, so prospective families should request current menus, sample meals, and documentation of dietary protocols during a visit. Activities appear to be a relative strength where implemented — pet visits and staff-supported programming were mentioned as meaningful for residents. Physically, the building is described as worn in places, with congested and cluttered hallways and a confusing layout that some found claustrophobic or uninviting; these factors can affect accessibility and day-to-day comfort.

Taken together, the pattern is of a facility with a strong caregiving ethos at the staff level but with systemic operational and environmental challenges. Families considering The Meadows at Ashland should plan an in-person visit to observe staffing ratios and cleanliness, ask specific questions about infection-control protocols, transportation and transfer procedures, billing transparency, and how leadership addresses family concerns. Request current inspection reports, staffing schedules, and examples of communication logs to help weigh the facility’s interpersonal strengths against the operational risks described in reviews.

    About The Meadows at Ashland

The Meadows at Ashland is a for-profit nursing facility located at 1700 Furnas Street in Ashland, NE, with 97 certified beds and an average of about 80 residents each day, and you'll find that they offer both skilled nursing and intermediate care for people who need help with daily living or medical needs, so they give different levels of care, like long-term care, short-term rehab, subacute care, memory care in two dementia care units, independent living, assisted living, adult day services, and hospice and home care, so it's a place with lots of options whether someone needs just a little help or full-time support every day. The staff provide about 3.65 nurse hours per resident each day, with registered nurses covering about 0.30 hours per resident, and there's always 24-hour nurse coverage, and they've got a high nurse turnover rate at 79.4%, and the staffing rating is low at 1.0 star by federal standards, so the amount and stability of staff might be a concern, with various deficiencies noted like nurse skills education and physician services, including two deficiencies tied to infection, and a total of 19 deficiencies in the latest report, and eight substantiated complaints about care or safety, and there's been one fine of $13,000, so these are things to consider when choosing a facility. The health inspection score stands at 2.0 stars with a weighted survey score of 46.0, where a lower score is better, and the quality measures also rate as 1.0 star, so while they do provide many types of care, there are inspection and quality marks to look at before deciding. The facility sits on lush, landscaped grounds with a big enclosed courtyard, a birdbath, walking paths, and seating all around, while inside you'll see welcoming resident rooms, spacious dining areas, common seating spaces with things like fish tanks, and memory care suites that feature single beds, floral curtains, and soft lighting. Residents can join in wellness and social activities, and an activities director plans those for everyone, so you'll see things like social outings, private meals, chef-prepared food, and group events, plus services like medication management, occupational, physical, and speech therapy, laundry, housekeeping, beauty and barber, and Wi-Fi, with meal and snack options in the dining room or in private, and transportation for outings or appointments. They have an emergency call system for safety, and people in secure memory care get extra help to keep them comfortable and protected, which is important for those with Alzheimer's or memory symptoms who need a special environment. The Meadows at Ashland is affiliated with Avid Healthcare Group and is officially registered as MEADOWS AT ASHLAND LLC, and you can expect an environment focused on "growing wellness rooted in compassion," so even though there are a range of support services and on-site amenities to help make daily life pleasant and safe, families will want to look closely at the ratings, nurse turnover, and prior report findings to see if it fits their needs.
