    Immaculate welcoming staff but cautious

I toured the facility and was immediately struck by how immaculately clean, organized, and well-kept it is - staff were welcoming, kind, professional, and residents seemed happy with activities, nutritious meals, and attentive rehab care. I felt my family member would be in good hands, but I also heard recurring concerns about understaffing and occasional medication/care lapses, so I'd recommend asking about staffing levels and medication protocols before deciding. Overall impressed but cautious.

    Summary review

Overall sentiment across the reviews is strongly mixed, with a majority of reviewers praising staff, cleanliness, rehabilitation services and the activity program, while a smaller but highly significant set of reviews describe serious safety, medication, and neglect concerns. Many family members and residents describe the staff as friendly, warm and family-like; nurses, aides and therapists are frequently singled out for compassion, patience and skill. Rehab outcomes are highlighted by multiple reviewers—examples include residents regaining mobility within weeks and positive short-term therapy experiences. The facility is repeatedly described as clean, well-kept and pleasant, with attractive grounds, visiting-friendly spaces and a calm admissions/check-in process. Activities, family involvement, spiritual care, attentive social work, and accommodating dietary services are other common positive themes.

However, an important and recurring counterpoint in the reviews is the presence of serious safety and quality-of-care complaints. Several reviewers reported medication errors and omissions—most notably missed glucose medication for days and insulin timing inconsistent with physician orders—one of which allegedly resulted in a diabetic coma and ICU admission. At least one review alleges a death and questions about accountability for missed cardiac medication. There are extremely serious accusations alleging neglect (no feeding, no water for fully dependent residents), concealment of residents in beds pushed against walls, and even claims that a resident died due to conditions described as "hellish." These reports also allege attempts by facility staff to cover up incidents and a need for families to call 911 to get appropriate care. Such accounts are severe and, while not the dominant theme numerically, represent critical safety concerns that prospective families must treat as high priority.

Staffing and management are another area of divergence. Many reviews praise dedicated, professional leadership, the Director of Nursing, named staff (for example, Cynthia, Priscilla and Mari), and floor teams that make residents feel secure. Several reviews call out excellent communication from staff, including frequent updates and a feeling that loved ones are in "good hands." Yet other reviews report severe understaffing, high turnover among caregivers, impatience or lack of empathy from certain nurses or supervisors, unprofessional conduct, and a lowered standard of care when staffing is inadequate. This contrast suggests variability by unit, shift, or time period—episodes of excellent staffing and leadership may coexist with shifts or times when staffing levels or staff competence drop, leading to adverse outcomes.

Dining and housekeeping impressions are also mixed. Multiple reviewers praise high-quality, tasteful meals, accommodating kitchen staff (even bringing in communion or special items like tea with fresh mint), and nutritious balanced food. Conversely, a portion of reviewers voiced food complaints and fellow reviewers reported unclean living quarters or unsafe conditions. Similarly, most comments describe the facility as immaculately clean, organized and pleasant, while a minority describe dirty rooms or unkempt conditions. These contradictions could reflect differing experiences across wings, timeframes, or expectations.

Activities, social engagement and the physical environment are consistently noted as strengths: an active calendar with games, trips and annual celebrations, friendly aides who engage residents, and well-maintained grounds. Many reviewers emphasize that residents appear happy and social, and that family visits are accommodated and pleasant. Praise for housekeeping, front-desk hospitality, and little touches (chocolates, large TVs, Easter brunch) reinforce the frequently positive daily-resident experience.

In summary, the facility shows many attributes families look for—compassionate staff, effective rehab, cleanliness, active programming and strong family communication. Nevertheless, there are a number of severe, specific complaints that cannot be overlooked: medication omissions and errors, alleged neglect resulting in hospitalizations and at least one alleged death, reports of cover-up attempts, and recurrent comments about understaffing and turnover. These serious negative reports appear less numerous than the positive ones but are of sufficiently grave nature that they materially affect the overall assessment.

If you are evaluating Peace Care St Ann's, weigh the positive experiences about staff, rehab and environment against the critical safety-related allegations. Recommended due diligence based on these reviews: ask the facility for recent inspection and health-department reports, inquire specifically about medication administration protocols, nurse-to-resident staffing ratios by shift, turnover rates, incident reporting and how they communicate and remediate adverse events. Visit multiple times (including evenings/weekends), speak with current residents and families, and request references from recent discharges to help confirm whether the highly positive experiences or the troubling safety reports better reflect current, typical performance.
