Staff provides round-the-clock support and supervision, and the community is set up for people who need extra help with medical care, therapy, or daily life, so people who stay here get access to skilled nurses, physical rehab, dialysis, and even an award-winning respiratory therapy wellness program, including a pulmonary rehabilitation and ventilator unit for those who need it. Residents benefit from structured programs that encourage socializing and making friends, along with a full schedule of social, educational, and entertainment events, and there's a focus on keeping everyone as active and comfortable as possible, with amenities like dining rooms and spaces for recreational activities. Bridgewater Center also offers dementia care, palliative and hospice care, respite care, and comprehensive care planning to help families navigate changing needs, so whether someone needs short-term recovery after a hospital stay or ongoing support for chronic conditions, the facility has a large set of clinical resources, including Cardiac ICU, Medical Surgical ICU, Pediatric ICU, Neonatal ICU, and Burn care, though it does not have PET or MRI services, nor trauma center services. The facility welcomes residents with Medicare, Medicaid, private pay, and most insurance, working with managed care plans and HMOs. Nurse practitioners like Maria Panlilio-Ortiz, NP, specializing in Geriatric Medicine, oversee primary care, while daily routines are designed to help everyone feel at home, so families can focus on their loved ones' well-being knowing skilled professionals are always supervising care.

Kind staff but facility unsafe
I saw both sides: many staff were kind, attentive and the rehab/activities could be excellent, but the place is chronically understaffed and mismanaged. Incessant alarms, noisy nights, filthy rooms/linens, unsafe medication handling, ignored call bells, bedsores/infections and poor communication made it unsafe. I would not trust this facility with a loved one until cleanliness, staffing and safety are fixed.
