    Understaffed facility, poor communication, decline

I liked the location, beautiful dining area, and generally clean common spaces, but I cannot recommend this place. Staff were kind at times, yet communication was atrocious - reception often unmanned, phones unanswered, and callbacks rare. The facility felt severely understaffed: slow responses to calls, rushed/shortened therapy, residents left in bed, inconsistent medical attention, and hygiene issues were evident. Visitation and activities were limited, administration was vague, and for the price my family member's condition regressed rather than improved.

    Summary review

Overall sentiment in the reviews is mixed but leans toward concern. Several reviewers praise the physical facility — describing it as clean, well-kept, with a nice layout, distinctive doors, a beautiful dining area, and an outdoor park for outings. Some families explicitly thank staff and report professional, respectful treatment and excellent medical care. The smaller size of the center is cited as a positive by some, giving a more intimate atmosphere and convenient location close to amenities.

However, a significant portion of reviews raise serious care-quality and safety concerns. Multiple accounts describe hygiene problems (soiled clothing left on residents, unaddressed sores, signs of possible infection), reports of missed hygiene and bathing (residents 'never showered'), and at least one allegation of a resident experiencing severe decline in mobility culminating in gangrene. There are also mentions of a resident developing a cold sore and at least one death during or after a stay. These reports suggest potential neglect and inconsistent clinical oversight in some cases.

Staffing and clinical service quality emerge as primary themes driving negative experiences. Many reviews report that staff are overworked and the facility is understaffed — too few nurses and CNAs to meet resident needs, leading to delayed medical attention, missed follow-ups, shortened or rushed therapies, and residents being left in bed for long periods. While some reviewers praise individual staff members as compassionate and professional, others describe unresponsive staff, unanswered phones, unmanned reception desks, and no callbacks from administration. The result described is inconsistent care quality: some residents receive attentive, dignified care while others suffer neglect or lapses in medical management.

Communication and administration are repeatedly criticized. Families report atrocious or vague communication from administration, difficulty reaching staff, and slow paperwork or transfers between floors. Several reviewers link poor communication to inadequate family engagement around activities and medical decisions. COVID-era restrictions and limited visiting hours are also mentioned as compounding family frustration; coupled with limited activity invitations and noisy activities adjacent to resident rooms, the social environment is described as lacking for some residents.

Facility features and amenities are a mixed bag. Positive remarks about cleanliness, dining areas, and safety features (such as exit-prevention buttons) contrast with negatives like large multi-bed rooms (four per room) that raise privacy concerns and complaints about availability of beds. Dining is described as "okay" by some — acceptable but not exceptional. The center’s smaller size and proximity to services are beneficial for some families but are also associated with limited capacity and occasional "shady" impressions or concerns about resources.

A clear pattern is variability: the same aspects are praised by some and criticized by others. Positive reports of professional, respectful care and clean, inviting facilities exist alongside troubling reports of neglect, poor hygiene, delayed medical care, and administrative unresponsiveness. The most frequently cited root cause for negative reports is understaffing/overwhelmed staff leading to inconsistent care and communication failures. Financial and availability concerns are also raised, with some reviewers perceiving that money or bed availability influences the level of care.

Bottom-line: The Plaza Rehab and Nursing Center appears to offer a pleasant physical environment with some capable and caring staff, but there are recurring, serious concerns about staffing levels, communication, clinical consistency, hygiene, and administrative responsiveness. Prospective residents and families should weigh the reported strengths (clean facility, good dining area, some excellent staff and care instances) against reports of neglect, delayed medical attention, and poor communication. If considering this facility, ask specific, documented questions about staffing ratios, infection prevention protocols, therapy scheduling, bathing/hygiene routines, call system availability in rooms, visitation policies, and how the facility communicates with families; request references and recent inspection reports to verify safety and quality measures.

About The Plaza Rehab And Nursing Center

The Plaza Rehab And Nursing Center sits at 100 West Kingsbridge Road in the Bronx and offers skilled nursing and rehabilitation care for seniors. The center has 816 certified beds, usually caring for about 726 residents every day, and has a for-profit partnership ownership under the name TCPRNC, LLC, managed by Leopold Friedman and Avi Philipson since September 2016. This nursing center is affiliated with Citadel Care Centers, and while it's connected with Jewish Home and Hospital Lifecare System, which is a nonprofit, the Plaza itself runs as a for-profit facility. The nursing home is one of three campuses, including sites in Manhattan and Westchester, and it's associated with the Sarah Neuman Center for Healthcare and Rehabilitation. The Plaza provides not only long-term care but also rehabilitation, sub-acute care, adult day care, and home health care services, with care plans meant to fit the needs of each resident. There's a state-of-the-art therapy center, a therapy room, an on-site dialysis center, and cutting-edge cardiac and sub-acute rehabilitation. They also provide social and community services, skilled nurses, doctors, other healthcare staff, and offer specialized nutrition and concierge nursing.

Residents can expect a Resident Council to help with communication and voicing concerns, and the facility tries to create a cheerful and caring environment with spacious rooms and fine dining. The CMS gives the Plaza Rehab And Nursing Center an overall 2-star rating, which is below average, with average health inspection ratings and much below average staffing ratings. The nurse staffing level is 2.95 hours per resident a day, and licensed nurse care totals 57 minutes per resident per day, which falls below state and national averages, as does the 21 minutes with a registered nurse and 36 minutes with a licensed practical or vocational nurse. Physical therapist staffing matches the national average at 5 minutes a day. The nurse turnover rate is 31.8%.

The quality of resident care for long-term residents is rated much above average at 5 stars, though short-stay measures are lower at 2 stars and the general quality of resident care is above average at 4 stars. The center reports some care strengths, such as lower-than-average rates of pressure ulcers and urinary tract infections among long-term residents, though more long-stay residents need increased help moving and with daily activities. The re-hospitalization rate for short-stays is a bit higher than the average. They provide most but not all flu and pneumonia vaccines, with the pneumonia vaccine rate being higher than average but flu vaccination just a little lower. There have been no recent federal penalties or fines, but CMS inspection reports have found deficiencies, including some concerning matters like failure to protect residents from abuse and incomplete care plans. The Plaza has also had inspection delays and complaints, with multiple deficiency reports on record.

Special features at the Plaza Rehab And Nursing Center include a focus on helping patients regain strength and quality of life, transitional care programs, on-site clinical care, and long-term rehabilitation services. The center is not a hospital or a continuing care retirement community but focuses wholly on nursing and rehabilitation needs for seniors, serving thousands of people daily as part of a larger system.
