    Excellent therapy, cleanliness, but unsafe

I visited a friend and found a very clean, well-organized facility with outstanding rehab therapists - my friend looked happy and many front-desk staff, CNAs and therapists were kind and attentive. At the same time I experienced unresponsive and inconsistent staffing: long nurse wait times, ignored call bells, missing therapy transport, delayed/inadequate pain relief (only one patch), medication/communication errors and occasional rude or discriminatory behavior, which felt unsafe. Overall: excellent therapy and cleanliness, but uneven staff quality and serious lapses - stay involved and advocate if you choose this place.

About Boro Park Center for Rehabilitation and Nursing

Boro Park Center for Rehabilitation and Nursing sits in Brooklyn, New York, and works as part of the Centers Health Care network, which folks know in the Northeast for post-acute care. The place has about 51 to 200 employees and is run as a public, for-profit company. Supervisors there keep a close watch, making sure staff provide attentive, professional support, and they're always working on new training and research, which makes their care up to date. Staff speak English, Spanish, Russian, and Arabic, which helps many residents feel at home. There are 27 certified beds out of 50 available in 2025, and the center accepts Medicare and Medicaid.

People come here mainly for short-term rehab after an injury, surgery, or illness, but the place also offers long-term care and services for people needing extra help with movement or serious medical needs. There's 12 to 16 hours of skilled nursing each day, with a 24-hour on-call system and round-the-clock supervision, so people always have the support they need. Nurses and aides help with daily needs like bathing, dressing, and medication, while therapy teams work to get folks stronger every day, often through personalized rehab plans including their RehabStrong™ program. There's care for adults over 60, inpatient rehab, and support for those with disabilities, thanks to a partnership with YAI Network.

The Boro Park Center handles a wide range of conditions, including cardiac problems, dementia, dialysis, HIV/AIDS, and people who need ventilator care or wound care. They work closely with area providers like Maimonides Medical Center Geriatrics and programs such as ChargePointCenters For Care. Medical staff stay on site and handle complex needs, with services that include pain management, IV antibiotic therapy, pulmonary care, stroke and orthopedic recovery, and respiratory management. There's even an Activities of Daily Living (ADL) suite so people can practice skills in a setting just like home, with its own bathroom, bedroom, kitchen, and laundry. Therapy options cover physical, occupational, and speech therapy, and a full set of health services is on hand, right down to lab and urgent care.

Residents live in rooms that come furnished, with private bathrooms, air conditioning, Wi-Fi, cable TV, kitchenettes, and telephones, and there are nutritious, tasty meals served daily. The environment feels warm and safe, with kind staff who try to make sure everyone feels welcome, and the center offers both short-term and long-term care, as well as day programs and assisted living options. Residents enjoy community activities-arts and music classes, movie nights, and groups run by fellow residents. The grounds have natural landscapes that help with rehab, and the facility uses modern technology for patient records and staff communication, so things run smoothly.

During COVID-19, Boro Park Center put safety first, following strict CMS and New York State guidelines, giving OTC COVID-19 rapid tests to visitors, allowing compassionate visits during serious decline, and providing PPE when needed. The nursing home meets industry standards, has a CMS Five Star Rating, and keeps a close community feel with many hands-on caregivers for each resident. People can feel confident that Boro Park Center has strong rehab programs, including GO Rehab, and many health and therapy services to help each person regain their strength and independence.
