Brooklyn Center for Rehabilitation and Nursing sits as a skilled nursing facility and provides a good range of care for seniors who need both short-term rehab and long-term nursing care, and folks always notice how they have what's called Ultra-Care, which means they're equipped for advanced and specialized care services, so there's a strong focus on healing with programs like the designated Wound Care service where wounds and skin injuries get close attention, and if someone's facing a tough recovery, the facility's got tailored rehab and therapy that's built for supporting people as they regain strength or just need to keep up their health. The TBI/Neurobehavioral Unit sticks out here, handling specialized neurobehavioral care for folks who need extra help with brain or behavioral health recovery, and the GO Rehab Program is part of their line-up, along with RehabStrong™, all of it coming together using the latest rehab technology and a staff of expert therapists who develop each rehab plan according to every resident's own needs. Brooklyn Center offers a big range of health services, including Cardiac Care for heart problems, COVID-19 Care, Dementia Care, Dialysis (with a home dialysis program set up for convenience), HIV/AIDS Care, Hospice, Orthopedic Care for bones and joints, Pain Management, Pulmonary and Stroke Care, Ventilator Care, and several other supports. The center also runs Adult Day Care, Assisted Living, Home Health Care, Urgent Care, and Skilled Nursing, plus lab and nursing home services-so some people just come for the day and some stay long-term. Every resident gets five-star accommodations and more than one hands-on caregiver per resident thanks to a large, tight-knit staff, and round-the-clock care is the norm, backed up by on-site clinical specialists who handle many health conditions. You'll see free bedside phone, WiFi, and TV, along with meals shaped by a Nutrition Program and a Wellness program, and the place makes a point to use the latest electronic health record technology so staff can communicate quickly about each person's needs. The building tries to keep things comfortable and refined, with amenities that help people settle in, and the environment often feels community-focused, with a recreation director leading social and group activities so residents can stay busy and connected. Brooklyn Center for Rehabilitation and Nursing is part of the Centers Health Care network, known in the Northeast for its post-acute care, and the team here tries to maintain a family-like atmosphere while offering many health services all under one roof.

Excellent therapy poor long term
I loved the bright, modern building and the rehab team - therapists got my mom walking again - but that's where the praise mostly ends. Staff responsiveness and professionalism were inconsistent: phones went unanswered, the social worker never returned calls, and the place felt chronically short-staffed. I saw serious care lapses - missed/prescribed pain meds, long waits in soiled diapers, inadequate bathing, safety/fall issues, and messy discharge/insurance coordination. In short: excellent therapy and facility, but I'd only consider it for short, closely supervised rehab - not long-term care.
