    Caring staff, excellent rehab, recommended

I had an amazing stay - the staff are caring and attentive, the nursing and rehab teams superb (they helped me walk again), and social services were responsive. The facility and short-term unit are clean, the gym is well-equipped, meals are good, and ongoing renovations look great. I felt safe and supported every day and would highly recommend this place for short- or long-term rehab.

    Summary review

Overall sentiment: The overwhelming majority of reviews present Premier Nursing and Rehab Center of Far Rockaway as a strong performer — particularly for short-term, rehabilitation-focused stays. Reviewers repeatedly praise the facility’s rehabilitation program (PT/OT/ST), describing therapists and the director of rehab as “superb,” “the best of the best,” and crediting the team with measurable, practical outcomes (e.g., helping patients regain mobility, walk again, or return home). Short-term rehab experiences are consistently described as successful, efficient, and goal-oriented; multiple reviewers explicitly recommend the center as the top short-term rehab option in the Far Rockaway area.

Staff quality and clinical care: Across reviews the staff receives near-uniform praise. Nurses, CNAs, therapists, social workers, and reception/frontline personnel are described as caring, kind, communicative, and attentive. Nursing leadership and administration frequently receive positive mention for responsiveness and involvement; many reviewers emphasize daily communication to families, timely medications, attentive wound care, and individualized attention. The presence of on-call doctors and a nurse practitioner is repeatedly noted as a reassurance and clinical strength. Several reviews single out wound care, organized care teams, and detailed nursing oversight as standout elements.

Facilities, cleanliness, and amenities: The facility’s physical environment is often described as clean, well-kept, and recently renovated — especially the short-term rehab unit, lobby, and first floor. Renovation-related comments are highly positive: reviewers report a modern, attractive short-term unit that is quiet, comfortable, and conducive to recovery. The rehab gym is a standout amenity mentioned in many reviews: described as well-equipped, clean, and integral to successful therapy. Safety features such as a secure entrance, easy check-in/drop-off, and a sense of peace and security are also commonly cited.

Dining, activities, and resident life: Many reviewers report satisfactory dining with customized menus and options, with the kitchen and dietary staff receiving commendations. Recreation and activities are also positively described: daily activities, patio and games, and staff-led programs contribute to a pleasant, home-like environment for residents. Several reviews indicate that residents are kept engaged, and families appreciate the recreation staff’s involvement.

Administration and management: Most reviewers praise administration as caring and responsive; some note that administrators take an active, hands-on approach. However, there are a small but important number of strongly negative comments about administration/ownership. A handful of reviewers allege mistreatment, dishonest behavior, or a lack of dignity and respect from management; a few mention new management with declining care quality or that the interior differed from photos. These negative reports are not the dominant theme, but they are significant because they contrast sharply with the otherwise positive consensus and warrant attention.

Negative patterns and caveats: While the vast majority of feedback is positive, several recurring concerns emerge. A few reviewers mention localized odors (bathroom residue) and a specific trash incinerator/drainage issue. Parking availability is a common practical concern. There are mixed comments about food quality — many praise the kitchen, while a minority report problems (some of which were reportedly addressed promptly). Renovations, while generally praised for improving the environment, have been noted as ongoing and occasionally disruptive. A small subset of reviews raises serious concerns: reports of mistreatment, confinement of non-ambulatory residents, and claims that the facility is “not for disabled” or has age-related suitability limitations. These more severe criticisms are isolated but should be investigated by prospective families.

Overall assessment and recommendations: Premier Nursing and Rehab Center of Far Rockaway appears to excel in rehabilitation services, staff attentiveness, cleanliness, and modernized short-term units. For families seeking short-term post-surgical rehabilitation or functional recovery, the facility repeatedly receives high praise and personal recommendations. Prospective residents and families should, however, do due diligence: ask about parking logistics, verify current renovation timelines, request a tour to check for any odors or drainage problems, and clarify dietary options if food variety is important. Given the minority of reports alleging mistreatment or management issues, families considering long-term placement should ask about management stability, complaint-resolution procedures, and recent quality metrics or inspection results. Overall, the dominant impression from reviews is of a rehabilitation-focused, staff-oriented facility with strong clinical outcomes and a generally positive, caring culture — with a small number of isolated but serious negative reports that merit direct inquiry during the admissions or touring process.

About Premier Nursing and Rehab Center of Far Rockaway

Premier Nursing and Rehab Center of Far Rockaway serves about 174 residents and has 183 certified beds, focusing on both short-term and long-term rehabilitation along with memory care and assisted living services, and you'll find things like physical, occupational, and speech therapy for folks with many different needs, whether it's mobility, breathing problems, injury recovery, Alzheimer's, or dementia. The center has a five-star nursing facility rating, and staff include Registered Nurses, Licensed Nurses, and Certified Nursing Assistants, with staff on hand at all hours, and it's managed these days by David Berov and Polina Khanina, with Robert Bleier and Benjamin Landa owning the place fifty-fifty. The staff sees themselves as dedicated and there are a lot of long-time employees who know the residents pretty well, with a Nurse Practitioner as part of the team and a Medical Director who's well-known in the community, plus there's always either a nurse practitioner or a physician assistant on hand to coordinate care to help folks stay out of the hospital as much as possible.

The center offers respite care for families needing a short break, has palliative care to help with stress and pain, and gives wound care, pain management, IV therapy, pre and post-operative care, and all sorts of nutrition help with things like dietary changes, tube feedings, or even intravenous feeding. For people on a dialysis schedule, the center's teamed up with a private dialysis clinic for more personal dialysis care, and when it comes to breathing problems, they provide oxygen concentrators, respirators, medications, and other medical equipment that helps make breathing easier, especially if someone needs a little more help with lungs or airways.

The first floor specializes in patients staying less than 90 days and the subacute rehab unit has its own up-to-date gym, dining, and recreational spaces as part of a recent $2 million renovation, and the rest of the building now has nicer bathrooms and common rooms, too. There's a social worker to help coordinate admissions, several room layouts for different budgets and needs, and a therapeutic recreation program to help folks get moving, try new activities, and stay connected-plus they hold religious services for different faiths and provide onsite parking for visitors, come rain or shine.

The center tries to keep residents connected to the wider world, and you'll see technology resources to teach seniors how to use cell phones, help with video calls, and handle new technology, which makes it a little easier to keep in touch with family and friends. Family is important here-they've set up the environment to feel more like home than a hospital, and there's a focus on social interaction and making everyone feel as comfortable and respected as possible.

Still, it's important to know the facility has a history of deficiencies-16 were found in recent inspections, including some for not having complete care plans, not keeping records private, and not always ensuring a safe or homelike space. The nurse turnover rate is about 44.4%, and on average, residents get 2.63 nurse hours per day from licensed nurses. They do listen to residents' concerns, using surveys and outreach to the community to try to improve. While people come here for care, support, comfort, and a range of medical services, the place does have room to grow in making sure every detail of care and cleanliness hits the highest standard.
