The Isabella Center for Rehabilitation and Nursing Care sits right in Washington Heights, upper Manhattan, where it's been part of the community since 1875, running as a non-profit and nonsectarian organization on its main campus at 515 Audubon Avenue. The center offers both short-term rehabilitation and long-term care, with a 705-bed nursing home, and has Isabella House, which is a 17-story building that gives about 250 independent adults a place to live, with 12 floors of furnished apartments, three floors for nursing care, and two floors for services and public spaces. People who stay at Isabella can get skilled nursing, memory care with a dedicated Memory Care Residence, specialized inpatient rehab, and support for activities of daily living and medication management. The center has had a long history, even winning a silver medal at the Paris Exposition of 1900 for the quality of its building and services, and over the years it's grown to welcome both men and women, no matter their race, creed, or nationality.

The facility also offers private rooms, amenities for comfort and safety, and a big list of activities and wellness programs to help with mental, emotional, and physical needs. There's a Senior Resource Center at 4026 Broadway, helping people with government benefits, health insurance, and other programs, plus a Home Care program that includes free Home Health Aide training at 5073 Broadway.
