    Excellent caregivers but poor management

I had a mixed experience at Park Gardens. I found the nurses, aides and social worker caring, communicative and great with therapy - they kept me/family updated and made me feel safe. But the facility had serious sanitation and cleanliness problems (urine smell), occasional rude or unavailable staff, long delays/poor communication, overcrowding and limited RN support. Overall, excellent frontline caregivers, but management, hygiene and staffing need major improvement.

    Summary review

The reviews for Park Gardens Rehabilitation & Nursing Center reveal a sharply polarized set of experiences, with a significant number of family members and residents praising the clinical care, therapy services, and several individual staff members, while other reviewers report serious concerns about facility cleanliness, building condition, staffing, and management responsiveness. Positive feedback centers on direct care and interpersonal interactions: many reviewers specifically call out strong physical and occupational therapy, knowledgeable nurses, compassionate aides, and an especially praised social worker who provides support, pre-arrangement information, and regular family communication. Several accounts describe proactive communication from staff, frequent updates on recovery, and successful rehabilitation outcomes (including reports of full recovery), which contributes to an overall sense of safety and well-being among those families.

Staff culture and caregiving are recurring strengths in the positive reviews. Multiple reviewers characterize the atmosphere as warm, friendly, and attentive; they highlight staff patience, empathy during difficult times, and a willingness to adjust care to changing needs. Numerous comments single out specific employees (nurses, aides, and the social worker) for exceptional performance and express clear recommendations. Activities and social engagement are also cited as positives — reviewers describe residents as having fun during activities and feeling respected and well cared for.

Contrasting sharply with those favorable reports are multiple reviews that raise operational and facility-level red flags. Several reviewers report poor cleanliness and sanitation problems, including an explicit mention of a urine smell and other sanitation incidents that led to a negative first impression. The building itself is described as in poor condition by some reviewers, and there are alarming statements about overcrowding: patients reportedly lying in hallways and rooms being overcrowded. Short staffing and limited RN coverage are noted as contributing factors to these issues, with some reviewers explicitly calling out times when staff were unavailable or support was insufficient.

Communication and management responsiveness show wide variability in reviewer experience. While many praised proactive family communication, others report poor or non-existent responses from management — including calls being hung up, placed on hold for extended periods (one review mentions being on hold for 15 days), and an inability to reach or identify a manager. These operational breakdowns exacerbate other concerns such as restrictive leave policies, difficulties contacting residents, and perceptions of inconsistent care across shifts. Food quality and medication management also produce mixed feedback: some reviews praise the food and nursing services, while others report bad food and concerns about residents being overmedicated.

Taken together, the review set suggests that Park Gardens offers solid clinical and rehabilitative care in many cases, supported by staff members who are attentive, communicative, and compassionate. However, there are recurring and serious concerns about facility cleanliness and physical condition, overcrowding, staffing adequacy, and management/communication reliability that lead to markedly different experiences for different families. The pattern indicates inconsistency — some families receive excellent, well-coordinated care and clear communication, while others encounter sanitation issues, limited staff availability, and operational lapses.

For prospective residents or family members, the key takeaways are to verify the unit/wing conditions and staffing levels at the time of placement, ask specific questions about sanitation practices and room occupancy, and clarify communication protocols (how and who provides updates, manager contact, and response expectations). If considering Park Gardens, it would be prudent to follow up in person, solicit recent references from current families, and confirm therapy schedules and the presence of dedicated staff (such as the social worker) who have been frequently praised in reviews. Management attention to cleanliness, staffing stabilization, and consistent responsiveness would likely address the most serious negative themes present in these reviews.

About Park Gardens Rehabilitation & Nursing Center

Park Gardens Rehabilitation & Nursing Center sits right in Riverdale, NY, where it helps folks with rehab and nursing care in a pretty straightforward way, being part of a bigger Continuing Care Retirement Community so people can stay as their needs change, and, well, they do have both long-term nursing care and short-term rehab for those coming out of the hospital, plus 200 certified beds, though about 17 were open in June 2025, and there's usually 183 residents each day. The staff helps with activities of daily living like bathing, dressing, and moving around, and they offer medication management, skilled wound care, IV therapy, and a good range of medical services including dentistry, psychology, and ophthalmology, while the nurse staffing runs about 2.99 hours per resident each day and they've got care coverage all day and night, though the nurse turnover rate is up at 22.6%, which can mean some different faces over time. The center accepts both Medicare and Medicaid, has bilingual caregivers (but mostly speaks English), and they offer a steady list of activities-movie nights, music, fitness and wellness programs, a game room, library, garden walkways, and resident-run groups-trying to keep things lively. Residents and families can join organized councils if they want a say in how things go, and you'll find an emergency system and supervising staff round-the-clock. There are some things worth noting, like deficiencies found in reports for not always making complete care plans or meeting new residents' needs right away, and inspectors have noted issues with protecting belongings and infection control, though no actual harm reported with those items, and the inspection process does keep the attention on these matters. The place makes therapy programs to fit individual needs, and tries to create a warm, home-like setting, with visits welcome daily from 10:30 in the morning to 8:30 at night, so family can be part of everyday life whenever possible. You'll find public transportation nearby, and if you need the home health services, those run from 8:30 to 4:30 and are right at 6677 Broadway. Park Gardens does aim to offer a real range of restorative therapies for people with different needs, and while they do their best to provide for folks in their care, they've got some areas needing attention too, which is really something to consider.
