Portland Health and Rehabilitation Center sits on SE Stark Street in Portland and holds 105 beds in a building that's gone through recent renovations to make things more comfortable and accessible. The facility stands out for its continuum of care, so residents can get help with daily needs, nursing care, physical therapy, and specialized memory care if they need it, and it doesn't matter if they're staying long-term or just for a short stay. There are skilled nurses on staff, and they're backed up by teams who help with speech, occupational, and physical therapies. Residents with Alzheimer's and other memory challenges can get extra support in units set up for different levels of care. The place keeps things going with group activity programs to help people socialize and keep busy, but folks can also spend time outside in landscaped spaces made for relaxing or being with others. It's a non-profit with decades in the community and a reputation for high standards, and over the years it's picked up recognition for good care. Residents get personalized treatment plans, nutrition checks, and discharge planning, and hospice care when it's needed. The staff speaks English, and everything is tailored to medical or therapeutic needs, whether that's rehabilitation, support services, or daily care. The center participates in both Medicare and Medicaid, so it can serve a wide mix of people. Part of EHC Management, it follows practices used by over 55 other nursing and assisted living facilities, and it stays up-to-date with modern amenities and inviting common rooms aimed at resident comfort and well-being.

Caring staff; unsafe, inconsistent long-term
I appreciated the genuinely kind, hardworking CNAs, nurses, therapists and activities staff (Malcolm and the activities director stood out) - my mother received tender rehab and died peacefully here. However, chronic understaffing, poor communication/shift-change disorganization, slow call-button response and reliance on agency staff led to unsafe, inconsistent care: missed/irregular treatments (oxygen forgotten, delayed breathing treatments and meds), mishandled wound care, and delayed hospital transfers. I also saw hygiene and maintenance problems, meal/dietary mismanagement, and belongings/room disorganization. For short, closely supervised rehab I saw good moments; for long-term care I could not fully trust them.
