Providence Elderplace - Gresham helps older adults get the care they need by offering many healthcare and support services all in one place. The main center sits at 17727 E Burnside St, Portland, and serves people all over the area, including Gresham, especially those aged 55 and older. Folks join the program while still living at home, with family, or in assisted living, so they can stay in their community longer and avoid nursing homes if possible. Providence Elderplace - Gresham is Oregon's only PACE program, covering Multnomah County and part of Washington County, and is part of a larger statewide network with a few other locations. Services focus on people with complex needs and help with groceries, social support, prescription drugs, equipment, and care planning, especially for those who don't want to leave their home. The community rating stands at about 3.6, and it features 39 different amenities, like movie nights, barber/salon, and community events, so there's usually something to do. The staff work as a team through a PACE model to make sure people's health, independence, and happiness come first, and they'll help families with long-term care, insurance, and more.
