    Caring staff, peaceful passing, recommended

I'm so grateful to the loving, caring staff who went above and beyond - comfortable rooms, good food, lots of engaging activities, and attentive medical care that kept my mom happy and helped arrange her meds, even during her peaceful passing. Staff were friendly and kind throughout. My only concern: an ambulance situation was mishandled and there was arguing over calling ambulance vs. hospice. Despite that, I highly recommend this facility.

    Summary review

Overall sentiment across the reviews is strongly positive, with repeated praise for the staff, the quality of care, and the atmosphere at Avalon Care Center - Scappoose. Multiple reviewers emphasize the friendliness of the staff and describe caregivers as kind, loving, and compassionate. A common theme is that caregivers often "go above and beyond," making residents comfortable and happy. The consistent use of words such as "amazing," "wonderful," "caring," and "grateful" suggests that family members and residents feel well supported and respected by the team on site.

Care quality and clinical support are highlighted as strengths. Reviews mention "very good care and medical service" and note practical assistance such as helping to obtain medications. There is also specific appreciation for the facility's handling of end-of-life needs: reviewers reported a "peaceful passing" and expressed gratitude for the manner in which staff managed those moments, indicating sensitivity and competence in hospice-adjacent care. These comments collectively point to reliable day-to-day care and attention to residents' medical and personal needs.

The facility environment and programming receive positive remarks. Reviewers describe the atmosphere as "home-like" with "comfortable rooms," and multiple notes reference "lots of fun activities" and an "engaging" activity program. This suggests a focus on social and recreational engagement that contributes to resident satisfaction and well-being. Dining is also mentioned positively: "good food" appears as part of the overall favorable impression, supporting the sense that daily living needs are being met in a pleasant way.

Despite the broadly positive commentary, there are a few noted concerns that deserve attention. One review reports mishandling of an ambulance and an argument over ambulance/hospice decisions. That incident stands out amid otherwise favorable feedback and indicates a possible weakness in emergency transport coordination or in communication between staff, families, and external providers during a crisis. Reviewers characterized this episode as mishandled and contentious, which contrasts with other accounts of calm, compassionate end-of-life support. This suggests the issue may be isolated but nonetheless important for management to review and address.

In terms of staff performance and culture, the reviews consistently portray a team that is attentive and emotionally invested in residents' well-being. Comments about staff being "helpful," expressing "love in every contact," and being "highly recommended" show a strong relational culture. Practical, tangible actions (e.g., assisting with medications) reinforce this perception and indicate both empathy and effectiveness.

Summary assessment and recommendations: Avalon Care Center - Scappoose appears to provide high-quality, compassionate care with a strong emphasis on resident comfort, engagement, and family satisfaction. Strengths include a caring staff, active programming, pleasant living spaces, and adequate clinical support. The primary area for improvement, based on the reviews provided, is emergency coordination and decision-making during urgent or end-of-life situations—specifically ambulance handling and clear communication about hospice transitions. Management should review protocols and communication channels for emergencies and hospice coordination to ensure that the positive, consistent care described in other reviews extends to crisis situations as well.

About Avalon Care Center - Scappoose

Avalon Care Center - Scappoose sits in a quiet neighborhood and offers skilled nursing home care with 40 certified beds, taking both Medicare and Medicaid, and serves an average of 26.7 residents each day, so it stays smaller than most places in Oregon. The staff includes registered nurses, nursing aides, and specialists who provide 24-hour licensed nursing care, general nursing, and dietary support, and if you ever need physical, occupational, or speech therapy, they have it on-site along with restorative and bariatric care programs, memory care, respite care for family caregivers, and even hospice care. People get both short-term rehab after hospital stays and longer-term care when they need more help with daily life, and residents use therapy like Applied Behavioral Analysis (ABA), Speech Language Pathology, Occupational Therapy, and social skills groups with a team including BCBA's, Speech Pathologists, Occupational Therapists, and Registered Behavior Technicians who help out. Residents and families can join councils to share ideas or concerns, and the facility tries to keep everyone safe and supervised, although past inspections noted some issues like safety from accidents and making sure food gets served right and stored safely, and the health inspection in August 2024 found four health and two fire safety citations, but no federal fines or payment denials in the past three years, which is something to note. They focus on quality assurance since their staffing and care ratings are good, with a 5 for overall quality, 4 for staffing, and strong scores for long-stay resident care, and you'll find comfortable bedrooms with outdoor views, dining spaces looking out onto a tranquil garden, and a big covered patio where people can spend time, along with daily group and solo activities and a beauty salon, all in a setting that tries to respect everyone's dignity and choices. The place keeps a calm, supportive community for both staff and clients, and encourages engagement with activities and therapy that aim to help with both physical and emotional well-being, offering all the services found in a continuing care retirement community, including Adult Care Home support, Assisted Living, Independent Living, Memory Care, and Skilled Nursing Facility care, which means you'll see people living there from different stages and needs. While the ownership is for-profit and runs under a corporation, the staff tries to create a welcoming space and the facility provides plenty of medical equipment, supplies, medication management, and dietary management with an on-site registered dietitian to support residents' day-to-day health, so people can settle in, know what to expect, and have access to support in a peaceful setting that's always open, every day of the week.
