    Kind compassionate staff, immaculate facility

I placed my dad in Unit B2 and was very pleased-kind, compassionate, professional staff (especially Jennifer and Joe Liberatore), an immaculate, spacious facility with lots of activities, and regular updates plus phone/video access that kept him engaged and smiling. We had minor hiccups-a sometimes unresponsive phone system and a few delayed/misplaced belongings-but the quality of care, safety (no COVID cases) and genuine staff attention gave me peace of mind; I would recommend and use them again.

    Summary review

Overall sentiment across the reviews for Lackawanna Health and Rehabilitation Center is strongly positive, with repeated emphasis on compassionate, attentive care and a clean, comfortable environment. Families repeatedly highlight the kindness and professionalism of nurses, CNAs, and therapists, saying staff go above and beyond to make residents feel at home and to maintain their dignity. Multiple reviewers reported peace of mind about their loved ones' care, explicitly recommending the facility and stating they would return for respite or hospice services.

Clinical care and daily hands-on support are consistent strengths. Reviewers specifically mention nurses and therapists taking good care of residents, clear improvements in resident communication (for example, a dad speaking more clearly), and comfortable, well-appointed rooms that are personalized with familiar photos and belongings. Hospice and end-of-life care are noted positively, with families feeling supported during those times. The patient business office and administrative staff receive praise for being patient and professional.

Operations, admissions, and family-facing coordination are another area of strength. Several reviewers singled out the admissions and marketing team — in particular, Joe Liberatore — for proactive planning, responsiveness, and personal assistance in setting up short stays and coordinating care. Families appreciated regular updates about resident status, especially during COVID-19, when the facility provided video chats and frequent status reports to maintain contact. Specific examples of family-oriented accommodations include phone access so relatives can hear loved ones, returned personal items like pictures, and staff who go out of their way to help with move-in and setup.

The facility environment and programming receive favorable comments: reviewers describe the building as immaculate and spacious, note that there are lots of activities, and say residents appear engaged and happy. Snack availability and weekly deliveries of goodies were repeatedly mentioned as small but meaningful touches that contribute to resident satisfaction and a homelike atmosphere.

Notwithstanding the many positives, there are a few recurring concerns. Several reviews mention visitation disruptions caused by COVID-19 restrictions — an external but impactful factor on family experience. Operational issues mentioned by multiple reviewers include delays in delivering personal belongings (items left at the desk and a cited three-day delay) and at least one report of an unresponsive phone system. There are isolated comments about unhelpful or rude staff and that staff can be busy, which suggests occasional lapses in responsiveness or customer service. These negative notes are far fewer than the positives but are important patterns for management to address to ensure consistently high family satisfaction.

In summary, Lackawanna Health and Rehabilitation Center is perceived by reviewers as a caring, well-run facility with strong clinical staff, proactive admissions/marketing leadership, and an environment that emphasizes resident happiness and comfort. The most frequently praised elements are the compassion and competence of caregiving staff, clear communication (especially during COVID-19), and the cleanliness and activity programming. The main areas for improvement are logistical responsiveness — particularly timely delivery of belongings and phone communication — and addressing the occasional reports of unhelpful interactions. Overall, the dominant theme is trust: many families feel their loved ones are in "safe hands" and would recommend or reuse the facility for respite or hospice care.

About Lackawanna Health and Rehabilitation Center

Lackawanna Health and Rehabilitation Center sits in Olyphant, Pennsylvania, and has been around for about 50 years, offering a range of health and rehabilitation services for seniors and others who need care, and the place used to be called Lackawanna County Rehabilitation Center, so you might still hear that name sometimes, but now they go by Lackawanna Health and Rehabilitation Center with Mr. James Miller as the administrator, and the center holds a BBB rating of A+. The facility has 272 beds and is set up to handle skilled nursing, long-term care, assisted living, independent living, and memory care, so people can move between levels of care as their needs change, which lets them age right in place if they want. They've got programs for subacute rehab, stroke, neurological recovery, dementia memory care including a locked unit for safety, and hospice care, and their staff provides around-the-clock nursing support along with help for daily tasks, medication, meals, wound care, and other medical needs.

Physical, occupational, and speech therapy are available every day, and outpatient therapy is offered for those who need it, plus there's specialized care for tracheostomy management, O2, nebulizer, CPAP, BiPAP therapies, along with diabetic, orthopedic, and oncology care, so they cover a lot of different conditions. Residents get nutritious meals cooked from quality ingredients, and the dining area aims to feel comfortable and home-like, while they also take part in activities to keep bodies and minds busy, including memory-focused programs, social gatherings, wellness events, and more, with both indoor and outdoor common areas, a fitness room, garden, library, beauty salon, computer center, gaming and wellness rooms, and group activities, making it easier to keep up with friends or find some quiet if needed. The community offers a maintenance-free lifestyle, housekeeping, and transportation for non-medical needs, and they help arrange outings and community-sponsored activities for residents who like to stay active.

Memory care residents have dedicated support and round-the-clock supervision, involving structured programs and memory-enhancing activities. Lackawanna Health and Rehabilitation Center also handles respite care, allowing folks to stay short-term so caregivers can get a break, and there's a temporary quarantine unit for non-vaccinated patients as an added measure of safety, with on-site Covid-19 vaccination available. Their clinical and wellness services focus on comfort, safety, and health, and they're set up to support people with a wide mix of needs.
