    Warm, homey, spotless, active community

I visited my aunt and was moved to tears by the warm, family-like atmosphere - spotless, homey rooms, lively activities like choir, and happy residents. The staff are professional, kind, attentive and organized; nursing, CNAs and therapy offer impressive follow-up and care coordination. I'm grateful for their compassionate, trustworthy care and would recommend this facility.

    Summary of reviews

Overall impression: Reviews portray Orwigsburg Nursing and Rehabilitation Center as a facility with a strong clinical and rehabilitative focus supported by a generally compassionate, professional staff. Many accounts highlight successful therapy outcomes, attentive nursing care, and proactive case-management practices that support discharge planning and post-discharge follow-up. The facility’s communal areas are frequently described as clean, bright, and welcoming, and its programming (choir, family-oriented celebrations, and other social activities) contributes to a home-like atmosphere that families often find reassuring.

Care quality and staff: Clinical care and therapy are consistently praised, with multiple mentions of improved mobility, increased independence, and effective treatment plans. Registered nurses and therapy personnel receive particular commendation for clinical competence and follow-through. At the same time, reviews indicate variability among direct-care aides and occasional short staffing, which can affect consistency of day-to-day assistance. There are also references to pay differentials between regular and agency aides, which may contribute to workforce variability. Overall, staff conduct is characterized as caring and professional, but operational staffing patterns can create uneven resident experience during some shifts.

Dining and personal-care responsiveness: Dining emerges as a recurring area of concern. Reported issues include inconsistent portioning, meal temperature control, and limited availability of specialty beverages for residents with dietary needs. Several families described bringing supplemental meals. Related to personal care, reviewers noted delays in responding to call bells and in toileting assistance; these are described as operational responsiveness problems rather than systemic clinical failures, but they affect resident comfort and dignity and merit attention.

Activities and atmosphere: Social programming and the facility’s effort to create a family-like environment are frequently noted as strengths. Choirs, celebratory events, and staff–resident interactions contribute to an inviting, personable atmosphere. Reviewers also appreciated inclusive hiring practices and the evident rapport between staff and residents during visits.

Facilities and management: The facility’s common areas and certain units are described as clean and orderly, but some reviewers observed dated rooms or decor in parts of the building. Administrative processes—especially admissions paperwork, business-office interactions, and Medicaid coordination—are called out as uneven; instances of difficult or prolonged paperwork interactions suggest a need for clearer upfront communication and more reliable handling of financial and insurance-related tasks. A few accounts also point to gaps in how the facility communicates with families after significant clinical incidents, indicating an opportunity to strengthen incident-response protocols and family outreach.

Recommendation and considerations: Many reviewers would recommend the facility, particularly for short-term rehab and for residents who benefit from attentive nursing and effective therapy. Prospective residents and families should weigh the facility’s strong clinical and social strengths against operational limitations: check the timing and responsiveness of direct-care coverage on the shifts that matter to you, ask about meal options and specialty dietary provisions, and clarify administrative workflows for Medicaid and billing during admission. Addressing these specific operational items with facility leadership before placement can help align expectations with the facility’s demonstrated clinical capabilities.

About Orwigsburg Nursing and Rehabilitation Center

Orwigsburg Nursing and Rehabilitation Center sits in a green, quiet suburban area, offering a place where folks can focus on getting better and feeling safe, and you see right away there's plenty of space to walk or look outside if you want. The center belongs to ProMotion Healthcare Services and works with Imperial Healthcare Group. They've got 130 certified beds, and on most days about 108 people live there. Staff use a team approach where nurses, therapists, and care workers pitch in, and every person gets a care plan meant just for their needs, which covers things like physical, occupational, or speech therapy, even for folks living with dementia. The therapists have lots of experience and training, and you'll find modern equipment throughout for rehabilitation and skilled nursing care. The nurses work every day and night, but their turnover rate is 59.3%, which is higher than the state's average of 47%, and the average nurse staffing hours are just under the state's average too, at 3.24 hours per resident per day compared to a state average of 3.9.

Orwigsburg Nursing and Rehabilitation Center carries a 5-star rating, but the facility has some flaws that came up in inspections. In the inspection report from July 26, 2024, inspectors listed six deficiencies, including one infection-related issue, and the total number of deficiencies comes to eleven. One problem was not following rules that protect against infection, and another was related to pharmacy services, where the staff didn't always reduce medication gradually or try other ways before giving medicine. Deficiencies also showed up in care for people needing help with urinary or bowel needs and for those who need support with daily activities.

Still, the center runs many programs for recovery and wellness, like RecoverWell, which aims to get residents as independent as possible, RestWell with rooms set up for peaceful and restful sleep, and #LiveWell to keep people moving and social. Rooms are private and comfortable, with meals prepared by chefs to fit special diets. Residents can use a beauty salon, enjoy the courtyards, take part in activities that touch on different cultures, religions, and education, and get both transportation and discharge planning when it's time to go home. Families can schedule tours and learn about care planning, and there's a 24-hour compliance line for reporting problems. The center also promises not to discriminate based on things like race, color, or disability, which is important for a feeling of community. Staff provide respite care, case management, and nutritional advice, and handle both long-term and short-term stays. So while Orwigsburg Nursing and Rehabilitation Center has many offerings and features to support both recovery and comfort, it's important to know both the good and the areas where improvements are needed.
