    Caring attentive therapy accessible recommended

I placed my husband here for respite care and have been very pleased. The staff are consistently caring and attentive, the therapy team is knowledgeable and compassionate, and management steps in effectively when needed. The facility offers social activities, is wheelchair-accessible, and provides excellent, reliable care-I highly recommend it.

    Summary of reviews

Overall impression: Reviews describe a facility with clear strengths in hands-on caregiving and therapy services alongside persistent operational and facility-level limitations. Families frequently note that clinical and rehabilitation staff are knowledgeable and compassionate, and that therapy programs produce positive functional outcomes. At the same time, reviewers raise recurring operational concernsโ€”particularly around staffing, responsiveness, and the physical condition of the buildingโ€”that prospective families should evaluate during a visit.

Care quality and clinical services: Many accounts emphasize attentive nursing and effective therapy/rehabilitation that contribute to measurable improvements and safe outcomes for residents. Therapy staff are described as competent and compassionate, and respite services are available. However, reviewers also describe instances of slower-than-expected responses to personal-care needs and call bells; these delays are tied by several families to potential short-staffing and inconsistent supervision, which can affect day-to-day resident monitoring and engagement.

Staff and family communication: Staff behavior and communication appear mixed. Numerous families praise particular caregivers for respectful, attentive interactions, while others describe concerns about tone, unreturned calls, and uneven communication from the team. Management has intervened successfully in certain cases, but perception of managerial professionalism and consistency varies. Families should ask about communication protocols, callback expectations, and escalation procedures when evaluating the facility.

Dining and household services: Review content included little consistent information about dining quality or meal service continuity. Given the limited commentary, prospective residents and families are advised to request sample menus, mealtime observations, and policies on special diets during a tour to assess whether dining meets individual needs.

Activities and social engagement: The facility appears to offer a range of social opportunities, including weekly Bingo, occasional shopping outings, and other events that provide resident engagement. Despite these offerings, some reviewers noted that individual residents experienced low participation or long periods of bed rest; assessing activity schedules and one-to-one engagement for less-mobile residents is recommended.

Facilities and cleanliness: A common theme is the buildingโ€™s age and room size. Rooms and bathrooms are described as small, and some units are shared with limited space for visitors. Cleanliness and odor concerns in common and resident areas were noted by multiple families; while some praise the overall environment, others recommend confirming housekeeping and sanitation protocols during a visit.

Management and notable patterns: Patterns in the reviews point to strengths in clinical and therapy care but recurring operational gapsโ€”especially around staffing levels, call response times, facility upkeep, and consistent family communication. Management has acted effectively in some situations, yet perceptions of managerial professionalism are uneven. Prospective residents should schedule an in-person tour, ask about staffing ratios and turnover, observe call-bell response during the visit when possible, review infection-control and housekeeping routines, and speak with therapy and nursing leadership about individualized care plans.

Bottom line: Oakhaven Nursing Center appears to deliver strong therapy and compassionate care from many staff members, with active social programming and a convenient location. Simultaneously, the facilityโ€™s aging infrastructure, small room sizes, intermittent cleanliness/odor issues, and inconsistent staffing and communication practices are substantive considerations. Families should verify staffing levels, observe responsiveness, inspect room size and visitor accommodation, and request details about amenities and sanitation standards before placing a loved one.

    About Oakhaven Nursing Center

Oakhaven Nursing Center sits at 123 Oak Street in Darlington, South Carolina, and has 88 beds for both long-term and short-term stays, serving seniors who need skilled nursing or intermediate care, so folks who are very frail or need help throughout the day can count on caregivers always being there since they've got 24-hour care services. You'll see studio apartments that start at $4,125 a month, and meals are provided through the therapeutic dining program, which means the kitchen can make meals for special diets as needed, and there's a focus on dining together, which feels pretty important for people in any kind of group home setting like this. Oakhaven has earned a 4.0 rating based on 7 reviews, with about two-thirds of the reviews giving 5 stars, which suggests many families and residents are pretty happy with the care and atmosphere.

They offer both assisted living and nursing home services, so if someone needs short-term rehab after a hospital stay, or longer-term support, the staff are trained for things like diabetic care, incontinence care, and helping people who can't easily get around by themselves. Residents who need therapy-like physical, occupational, or speech therapies-can get these right at the facility, and they try to keep people active with music, art and crafts, and exercise classes, both in the indoor common areas and outdoors where you'll find a courtyard, landscaped garden, picnic tables, and benches for sitting outside. People who want to be involved can join activities either on-site or when there's something planned off the property. Families mention the place is known for being clean, having good nursing care, and paying attention to special needs and requests, so that's something to keep in mind if you're looking for a place with a reputation for treating people with respect and care. Stacy Boatwright manages the staff, and there's a full senior care program in place to help each resident feel as at home as possible, whether they plan on staying for a while or just need some extra help recovering before heading home.
