    Neglectful, unsafe care; systemic failures

    I moved my loved one here for rehab and appreciated the friendly front desk (Vanessa) and exceptional therapists (Sarah, Sam) - a few nurses and aides (Annette, Crystal, Kia, Megan, Patricia) were genuinely caring. That said, I experienced chronic understaffing and distracted staff, slow or missed meds (including insulin), medication errors, cold/late meals, unanswered call bells for long periods, and unsafe care leading to falls, bedsores and even a death. The place often smelled of urine/mold, bedding was left wet or soiled, and hygiene/cleaning were inconsistent. Communication and administration were poor - missed meetings, wrong discharge notices, billing confusion and unreturned calls. Some staff are wonderful, but overall I found the facility neglectful and unsafe; I will personally monitor care and cannot recommend it until management fixes these systemic problems.
