Seneca Health and Rehabilitation Center, located at 140 Tokeena Road in Seneca, South Carolina, offers skilled nursing and rehabilitation care for seniors who need either short-term therapy or long-term care, and the building itself has a nice campus set up to help people relax and recover. Residents stay in suites they can personalize with their own belongings, and the whole facility offers amenities made for comfort, including daily activities, delicious meals worked out with help from a registered dietician, housekeeping, free laundry, cable, and Wi-Fi, and there's a therapy gym as well. The center has 132 certified beds and usually has about 111 residents a day, and both nurses and nursing assistants get assigned to residents in a steady way to make sure people get to know who's caring for them. A whole team works here, with doctors and specialists including orthopedics, cardiology, neurology, infectious disease, gastroenterology, pulmonology, and wound care, plus social workers, psychologists, and psychiatrists to help with mental health needs. There's physical, occupational, and speech therapy using new equipment, and the place helps with post-acute care, wound treatment, IV care, stroke recovery, Parkinson's support, dementia and Alzheimer's care, and respiratory care for things like cancer or muscular dystrophy or apnea, and there's help with pain management, hospice, and respite services too. They also help residents and families with Medicaid applications and have specialists to support everything from nutritional needs to medication management. Staff provide 3.42 nurse hours per resident per day, and there's a nurse turnover rate of 52 percent, with 24/7 skilled nursing always on site. Activities, social events, and educational projects happen daily to help keep people active and engaged. While the facility is accredited by The Joint Commission and aims for a family-oriented and healing-focused environment, the latest federal inspection found 16 deficiencies, including issues with menu planning, food storage and preparation, and infection control-there were three infection-related deficiencies, though inspectors didn't find actual harm but did see potential for it. The center is for-profit and owned by Scsnf Investment Group LLC, and other groups like Arba Four Holdings LLC and Bhgsc Holdings LLC have indirect ownership. Jimmy King is the administrator. More information is found at their website, https://senecahrc.com. There isn't much else available about the facility, but it remains a place focused on offering different types of care for seniors who need extra help with their health, recovery, and daily life.

Caring staff, inconsistent care; advocate
I've seen both sides: warm, family-like staff, attentive CNAs, excellent therapists, a clean facility and a hands-on new administration that's making real improvements. Care is inconsistent though-I've witnessed medication/dietary errors, some rude or unprofessional staff, turnover and occasional safety/neglect lapses-so you need to stay involved and advocate. Overall I recommend cautiously: great people and rehab when things click, but insist on close oversight.
