    Compassionate staff, serious safety concerns

I found a mix of excellent, compassionate caregivers (Evie, Nashita, Rhonda, Terry, Joahn, Hannah) and a clean, activity-filled facility with good therapy and personal attention that made my mother comfortable. Sadly, serious safety and staffing problems undermined that - disconnected/dirty oxygen equipment, missed meals, a UTI and hospital readmission, threats by a CNA and police involvement. Administration and some nursing were unresponsive or misleading, activities/family communication inconsistent. I'm grateful for the kind staff but, given the safety issues, I cannot recommend returning without major changes.

    Summary review

Overall sentiment for White Oak Manor York is mixed but strongly polarized: a substantial number of reviews praise the staff, individualized attention, and clinical services, while several reviews report serious safety and administrative issues. Many families describe deeply compassionate caregivers who provided emotional support, made residents comfortable in final days, and went beyond routine duties. Specific staff members were named and lauded for exceptional care, which suggests that individual caregivers can have a significantly positive impact on resident experience. Multiple reviewers highlighted a family-like atmosphere, frequent nurse updates (in some cases), comprehensive therapy services (PT/OT/SLP), dementia-aware care practices, and dietary accommodations for residents with swallowing issues.

Care quality and clinical services appear to be strong in many cases: therapy and rehab services are available and noted positively, some reviewers received timely and informative nursing communication, and regular doctor visits were mentioned. Several reviews emphasized clean rooms, hallways, and consistent laundry service, along with appealing meals and individualized diet accommodations. For families seeking dementia-capable care, reviewers noted staff familiarity with dementia-related needs and inclusive activities directed at engagement.

At the same time, a number of reviews report significant problems that are not minor inconveniences. There are accounts of unsafe care events and clinical lapses — for example, an oxygen line reported as disconnected, a dirty oxygen machine filter, a urinary tract infection leading to hospital readmission, and a resident fall requiring rehab. These incidents raise concerns about clinical oversight, equipment maintenance, and infection prevention practices. Some reviewers described nurses leaving rooms or inadequate response to changing medical needs, and at least one review explicitly stated they would not return because of poor care.

Management, communication, and consistency are recurring themes in the negative feedback. Several families experienced limited or inconsistent communication (few FaceTime calls, restricted in-person visits, or limited family updates) and cited unhelpful activities staff. More serious administrative complaints include allegations of dishonesty, poor responsiveness by management, and even active threats by a CNA that required police involvement. Multiple reviews explicitly mention staffing shortages or variability, leading to widely inconsistent experiences depending on shift or caregiver.

Facility condition and services are described in mixed terms: many reviewers praise cleanliness and laundry, but the building is repeatedly described as older and in need of updates. Dining receives mostly positive comments regarding taste and accommodation, yet there are isolated reports of meals not being delivered as ordered or other kitchen issues. Activity programming is frequently described as extensive and inclusive, but some reviewers reported particular activity staff who were unresponsive or unhelpful, suggesting variability within departments.

Patterns suggest that while White Oak Manor York can deliver high-quality, compassionate care — especially when staffed by the praised individuals — there are also important and potentially serious lapses in safety, administration, and consistency. Prospective families should weigh the strong positive testimonials about individual caregivers and therapies against reported clinical incidents and administrative concerns. Recommended due diligence includes asking about staffing ratios, clinical oversight (especially for oxygen and infection control), equipment maintenance protocols, visitation and family communication policies, fall-prevention programs, and how the facility addresses complaints and staff misconduct. Visiting the facility in person, meeting the care team (including those named positively in reviews if possible), and obtaining recent inspection or incident reports will help form a clearer, current picture of the facility's reliability and safety.

    About White Oak Manor York

White Oak Manor York sits at 111 S Congress St in York, South Carolina, and has served seniors since 1967 as a family-owned retirement community, with 109 beds for residents who may need skilled nursing care, short-term rehab, or different levels of assistance, so you'll find options like independent living, assisted living, memory care, nursing home services, and even outpatient rehab all in one place, which helps when care needs change over time. The nursing staff includes registered nurses, licensed practical nurses, and certified nursing assistants, with 24-hour supervision from physicians and an interdisciplinary team, and the care goes beyond simple nursing, since there's wound care, dementia care, balance and fall training, prosthetics and orthotics support, even dysphagia therapy for swallowing issues. Activities run meaningfully all day, no matter your mobility; folks confined to bed might enjoy iPad games, music, magazines, spiritual visits, or see family and friends, while group things like outings, arts and crafts, and cooking groups let everyone connect.

The community's big on keeping things social and lively, with recreational, social, and cultural programs, plus a full calendar of activities to encourage friendships and learning-there's a library, a barbershop and beauty salon, a community room, and amenities like laundry, van transportation for outings or appointments, and housekeeping. Therapies are a strong focus, with services in physical, occupational, and speech therapy available seven days a week, using programs like Accelerated Care Plus and Safe Transitions, and treatments can cover all sorts of rehabilitation, whether it's for stroke, back injury, amputees, or more general needs. Unique features include an ADL Kitchen for daily living practice, electrical stimulation, fluidotherapy, diathermy, ultrasound, and peripheral neuropathy therapy-these are technical therapies you don't see everywhere. Staff also includes medical consultants, social workers, dietitians, and therapists, and there's help with activities of daily living like bathing, dressing, eating, or medications.

The place can store and hand out medicines, manage complex care, and meets federal standards for Medicare and Medicaid, so more folks can use its services. Services run all day and night, including dental, podiatry, ophthalmology care, pharmacy, and even mobile X-rays or religious services. Family and caregiver education matters here, and there are transition and safety programs in place. The administrator is Christie Parker, and the overall approach centers on developing a personalized plan for every resident, so people get the right mix of health care, rehab, social support, and independence where possible. White Oak Manor York tries to make sure seniors have choices, connections to others, and help right when they need it.
