Covington Care and Rehabilitation Center, also called Covington Heights Care and Rehabilitation Center, is a skilled nursing facility in Sioux Falls, South Dakota, at 3900 S Cathy Ave. Licensed for 73 beds, the center had a closure planned for August 5, 2019, with 65 beds occupied as of the end of May that year, and ownership under Black Hills Receiver LLC decided on the closure for the residents' best interest, so there was a detailed plan to help everyone move smoothly. The center served older adults and offered Medicaid and Medicare financial services, and staff spoke English. Care at Covington Care and Rehabilitation Center included daily skilled nursing, therapy, medication help, and other support for seniors needing round-the-clock supervision in a safe setting with supervision and a 24-hour call system for help, plus safety features in each apartment like full security and grab bars. Rooms had private bathrooms, cable TV, kitchenettes, telephones, air conditioning, furnished units, and access to Wi-Fi. Housekeepers did laundry and apartment upkeep, and move-in coordination services were part of the process. Residents could use transportation and parking on-site, and there were walking trails, outdoor spaces, garden areas, and paths for exercise and relaxation, plus special occasion parties from time to time that helped keep everyone connected. Programs included daily scheduled activities, library access, music and arts rooms, a community movie theater, game room, and lots of ways to stay mentally active and engaged, from bingo and card games to arts and crafts classes and regular health monitoring. Fitness and wellness programs ran alongside spa and sauna access, fall prevention efforts, family support groups, and massage therapy options. Dining services included a community dining room, allergy-sensitive foods, restaurant-style meals, and diet help for diabetes or special needs, with a focus on nutrition, taste, and quality. There were also routines for regular physician assessments and ongoing health checks. The care team handled bathing, dressing, moving around, and custom medication management, with specialized caregivers available for Alzheimer's care in memory care environments designed to reduce confusion and prevent wandering. Apartment options included one-bedroom suites with all the main safety features, and the pet-friendly community supported personal requests and concierge help for resident needs. Amenities like balconies and outdoor sitting spaces let residents enjoy fresh air, and the routine of community activities helped foster a sense of belonging. At one point, Covington Heights received the Best of Senior Living All Star Award, which means residents and family members found their services worth recommending. No new residents are being accepted now due to the planned closure.
